Objective: This study is mainly to observe the changes in the tendon-bone healing interface after the anterior cruciate ligament reconstruction with autologous semitendinosus tendon in rabbits model by using the method of tonifying the kidney and activating blood circulation,and further demonstrate the mechanism of the method of tonifying the kidney and activating blood circulation in the tendon-bone healing after the anterior cruciate ligament reconstruction with autologous semitendinosus tendon in rabbits model,providing a basis for clinical experimental research.Methods: In this experimental study,48 healthy New Zealand white rabbits were selected and divided into four groups using a random number table method.The preoperative examination showed that the knee joint activity was normal,without redness,swelling,or deformity.Both the anterior drawer test and the Lachman test were negative.The anterior cruciate ligament reconstruction model was established using rabbit autologous semitendinosus autograft.The postoperative anterior drawer test(-)and the Lachman test(-)were used.The different postoperative groups were respectively divided into the kidney invigorating and blood activating group,the kidney invigorating group,the blood activating group,and the blank control group,Four white rabbits were randomly killed at the 4th,8th,and 12 th weeks after operation by gavage with different doses of 9 ml/(kg.d)of different drugs.The biomechanical tests of the structure of the femoral ligament tibial complex and the histological observation of the bone marrow tendon-bone healing interface were performed.Results: In this experimental study,four groups of rabbit models of anterior cruciate ligament reconstruction with autologous semitendinosus muscle were used.In the anatomical observation at the 4th and 8th weeks after surgery,the kidney tonifying and blood activating group had the best healing of the medullary orifice,synovial membrane coverage,and ligament tension,followed by the blood activating group,which was superior to the kidney tonifying group,and the blank control group had the worst.In the gross anatomical observation at 12 th week,the group of tonifying the kidney and activating blood circulation had the best healing of the medullary orifice,synovial membrane coverage,and ligament tension,followed by the group of tonifying the kidney,which was superior to the group of activating blood circulation,and the blank control group was the worst.At the fourth week,the ultimate tensile force values of the kidney tonifying and blood activating groups,kidney tonifying and blood activating groups were all better than those of the blank control group,with a statistically significant difference(P<0.05).At the fourth week,in a pairwise comparison of the four groups of ultimate tensile force,the kidney tonifying and blood activating group had the best ultimate tensile force for complete ligament rupture after reconstruction,the blood activating group was superior to the kidney tonifying group,and the kidney tonifying group was superior to the blank control group.At 8th week,the ultimate tensile force of the complete rupture of the reconstructed ligament in the kidney tonifying and blood activating group,kidney tonifying and blood activating group,and blood activating group were superior to the blank control group,with a statistically significant difference(P<0.05).In a pairwise comparison at the 8th week after surgery,the ultimate tensile force in the kidney invigorating and blood activating group was the best,the blood activating group was better than the kidney invigorating group,and the kidney invigorating group was better than the blank control group.At the 12 th week after operation,the ultimate tensile force of complete rupture in the kidney invigorating and blood activating group,kidney invigorating and blood activating group,and the blood activating group were all superior to the blank control group,with a statistically significant difference(P<0.05).After 12 weeks of operation,the limit tension was compared in pairs,and the kidney tonifying and blood activating group was the best,the kidney tonifying group was superior to the blood activating group,and the blood activating group was superior to the blank control group.In this experiment,the final states of the reconstructed ligaments in the four groups during the maximum tensile load test were intramedullary detachment,intramedullary rupture,and extramedullary rupture.Most of the kidney tonifying and blood activating groups,kidney tonifying and blood activating groups were extramedullary rupture,while most of the blank control group were intramedullary detachment.There was a statistically significant difference between different groups(P<0.05).Histological observation: In H-E staining,the growth status of fibroblasts and chondrocytes in the tendon bone healing interface of the kidney invigorating and blood activating group was the best at three different postoperative periods.At the 4th and 8th weeks after surgery,the kidney invigorating and blood activating group was superior to the kidney invigorating group,and at the 12 th week,the kidney invigorating and blood activating group was superior to the kidney invigorating group.The blank control group was the worst at all three time points.Masson trichrome staining showed that the growth of collagen fibers in the tendon bone interface kidney invigorating and blood activating group was optimal at three time nodes.At the 4th and 8th weeks after surgery,the blood activating group was superior to the kidney invigorating group,and at the 12 th week after surgery,the kidney invigorating group was superior to the blood activating group,while the blank control group was lower than the other three groups.Conclusion: Kidney tonifying and blood activating herbs can effectively promote tendon-bone healing after anterior cruciate ligament reconstruction in rabbits,which is superior to the efficacy of single kidney tonifying or blood activating herbs,and enhance the anti traction strength of the graft.
