Compared with traditional open surgery,minimally invasive surgery has the function of reducing surgical incision and scar,shortening recovery time,reducing bleeding and complications.The familiarity and operation proficiency of surgeons with the surgical operating system will greatly affect the effectiveness of the surgery.It is necessary to form a more effective evaluation method of surgical operation skills.At present,the evaluation of surgical skills at home and abroad mainly adopts expert evaluation,which is highly subjective and has not yet formed a systematic and quantitative evaluation method.The type of information collection is single,and the evaluation of human operational ability is one-sided.Therefore,this study proposed a comprehensive evaluation method of minimally invasive surgical operation skills,which combines the objective evaluation of motion skills and muscle fatigue with the subjective evaluation of self-evaluation.The specific work is summarized as follows:Firstly,it is the study of the training tasks design and evaluation methods.According to the complexity theory,a complex multistage star-like network system is built,which can provide a variety of scenarios and training tasks with various difficulties for surgeons with different operating levels.It is introduced into the field of minimally invasive surgery,and a basic training task and an advanced training task are designed,and the task complexity score is quantified.The methods of motion skill evaluation,upper limb muscle fatigue evaluation and task perception evaluation are introduced.Additionally,the experimental procedure is planned,which lays a foundation for the follow-up experiment operations and data analysis.Secondly,it is the study to evaluate the surgical operation skills of operators from the dimensions of “physics”,“physiology” and “psychology”.In the aspect of motion skill evaluation,the evaluation indexes such as completion time,speed,jerkiness and so on are selected,and the experimental results of novices and residents and different complexity are compared and analyzed.In the aspect of fatigue evaluation of upper limb muscle,the average rectified value and mean power frequency of EMG signal are used as evaluation indexes to analyze the fatigue degree and fatigue muscle difference of different complexity tasks.In terms of task perception evaluation,the characteristics of the operation on subjects in the experimental procedure are analyzed through the results of the scales.Finally,a comprehensive evaluation model of surgical operation skills is established.Based on the three criteria of “physics”,“physiology” and “psychology”,a hierarchical model is established,and the correlation coefficient method is used to screen the indicators.Then,the weight of each index and criterion layer are determined by the combination of analytic hierarchy process and entropy weight method.According to the fuzzy evaluation method,the relevant data are corresponding to the fuzzy index.Finally,a fuzzy comprehensive evaluation model which can be used to evaluate the operation skills of operators is established,and the comprehensive evaluation results are analyzed.In this study,the establishment of subjective and objective comprehensive evaluation model of operation skills in minimally invasive surgery is completed,which improves the training quality and the safety of surgical operation. |
