| Objective:The study compared the clinical characteristics of hyperlipidemic acute pancreatiti-s and non-hyperlipidemic acute pancreatitis in order to guide clinical diagnosis and tre-atment and benefit patients.Methods:The research used retrospective analysis methods by Collecting 120 patients with hyperlipidemic acute pancreatitis(HLAP)treated in Shenzhen Hospital of Chinese Academy of Sciences(Guangming)and 127 patients with non-hyperlipidemic acute pancre-atitis(N-HLAP)(N-HLAP group,B group)from 01 January 2017 to 31 August 2022(HLAP group,A group).32 patients of Group B who diagnosed biliary acute pancreatitis(BAP)were divided into Group C(BAP group,C group).The study compared the clinical data of patients in the N-HLAP group,HALP group,and BAP group,which included age,gender,body mass index(BMI),hospital days,combined diabetes mellitus,white blood cell(WBC),hypersensitivity C-responsive protein(hsCRP),triacylglycerol(TG),total cholesterol(TCHO),D-Dimer(D-D),serum amylase(SAMY),urinary amylase(UAMY),glucose(GLU),serum calcium(Ca2+),alanine transaminase(ALT),aspartate transaminase(AST),total bilirubin(TBIL),direct bilirubin(DBIL),local complications,recurrence rate and severe rate(Based on the APACHEII score)and then used statistical software to Process and compare and analyze the data.Results:1.The age of patients in the HLAP group was lower than that in the N-HLAP grou-p and BAP group and the difference between three groups was statistically significant(P < 0.05);There was no significant difference in the male incidence rate between the HLAP group with the N-HLAP group(P = 0.085 > 0.05),The HLAP group patients ha-d a higher incidence of the diseases in men than the BAP group patients,and the differ-ence between three groups was statistically significant(P <0.05).The proportion of co-mbined diabetic patients in the HLAP group was higher than the N-HLAP and BAP gr-oups,The difference was statistically significant(P <0.05).The HLAP group patients had higher BMI values than the N-HLAP group patients and the difference between thr-ee groups was statistically significant(P=0.014 < 0.05),There was no significant differ-ence of the BMI values between the HLAP group with the BAP group(P = 0.836 > 0.05);There was no significant difference of the hospital days between the HLAP group wi-th the N-HLAP group and the BAP group(P > 0.05).2.The mean values of blood white blood cells(WBC),triacylglycerol(TG),total ch-olesterol(TCHO),glucose(GLU)in the HLAP group were higher than those in the N-H-LAP group and BAP group and the difference is statistically significant(P <0.05).The blood amylase(SAMY),urinary amylase(UAMY),blood calcium(Ca2+),glutam-ate transaminase(AST),alanine transaminase(ALT),total bilirubin(TBIL),direct bilir-ubin(DBIL)of he HLAP group were lower than those in N-HLAP group and BAP gro-up and the difference is statistically significant(P < 0.05).There was no statistically sig-nificant difference in the mean values of hypersensitive C-reactive protein(hs-CRP)a-nd D-D dimer(D-D)between the HLAP group and the other two groups.3.There was no statistically significant difference in the severity rate between HL-AP group patients and N-HLAP group and BAP group patients.The recurrence rate of p-atients in HLAP group was higher than those in N-HLAP group and BAP group,whic-h had a statistically significant difference(P < 0.05).The proportion of patients with pul-monary infection in HLAP group was higher than those in N-HLAP group and BAP gr-oup,,which had a statistically significant difference(P <0.05).There was no statisticall-y significant difference(P> 0.05)to compare the proportion of patients with pleural ef-fusion,abdominal effusion,intestinal obstruction and peritonitis in HLAP group with t-hose in the N-HLAP group and BAP group.There was no statistically significant diffe-rence(P> 0.05)to compare the proportion of patients with local comorbidities in HLA-P group with those in the NHLAP and BAP groups.Coulsion:Patients in HLAP group were younger、fatter and had a higher rate of male incide-nce and combined diabetes than patients in the N-HLAP group and BAP group.The le-vels of triglyceride and total cholesterol were higher and the levels of serum and urine amylase were always lower than those in N-HLAP group and BAP group.The recurren-ce rate in HLAP group was higher than the other two groups.The recurrence of pancre-atitis was mainly related to poor control of lipid in patients and active and effective lipi-d control would help to reduce the probability of recurrence. |
