| Objective:To explore the clinical efficacy of liraglutide in the treatment of type 2 diabetes mellitus patients complicated with peripheral neuropathy,especially focusing on its effect on the improvement of peripheral nerve function.Methods:The study selected patients with type 2 diabetes mellitus and peripheral neuropathy who were diagnosed and treated in Luohu District People’s Hospital from October 2020 to September 2021,68 patients were enrolled.All Cases were analyzed retrospectively,28 patients treated with liraglutide and hypoglycemic management with exercise diet were selected as the observation group,and 40 patients treated with premixed insulin(insulin aspart 30)and hypoglycemic management with exercise diet were selected as the control group.After 20 weeks of treatment,some indexes between the two groups were observed and compared before and after treatment,which included the blood glucose level(FPG、2h PG、Hb A1c),inflammatory index(CRP,IL-6)level,oxidative stress index(SOD)level,the incidence of adverse reactions during the treatment,and the improvement of sensory nerve conduction velocity before and after the treatment.Results:(1)There were no significant differences in age,gender,course of disease,BMI,retinopathy,ASCVD,blood pressure,lipid levels and urine microalbumin/creatinine ratio between the two groups before the treatment(P>0.05).(2)There was no significant difference in the levels of FPG,2h PG and Hb A1 c between the two groups before the treatment(P>0.05).After the treatment,the levels of FPG,2h PG and Hb A1 c in both groups were lower than those before treatment(P<0.05);The levels of FPG and Hb A1 c in the observation group after treatment were slightly lower than those in the control group,and there was no significant difference between each groups(P>0.05),but 2h PG level in the observation group after treatment was significantly lower than that in the control group(P<0.05).(3)There was no significant difference in the levels of inflammatory indexes(CRP、IL-6)between the two groups before the treatment(P>0.05).After the treatment,the levels of CRP and IL-6 in both groups were lower than those before the treatment,and the index levels in the observation group were significantly lower after the treatment than that in the control group(P<0.05).(4)There was no significant difference in the level of oxidative stress index(SOD)between the two groups before the treatment(P>0.05).After the treatment,the SOD level of both groups increased significantly compared with that before treatment(P<0.05),and the observation group was higher than that of the control group(P<0.05).(5)During the treatment of the two groups of patients,1 patients in the observation group suffered some reaction of the stomach and intestines,which could be relieved by themselves without any treatment;In the control group,there were 2patients who experienced hypoglycemia(with an empty blood glucose of less than3.9mmol/L),but they did not suffer obvious symptoms.After adjusting the insulin dose,their blood glucose was improved,without causing any adverse consequences.(6)Before the treatment,there was no significant difference in the sensory conduction velocity of the upper limb nerve(median nerve and ulnar nerve)and lower limb nerve(common peroneal nerve and tibial nerve)between the two groups(P>0.05).After the treatment,the sensory conduction velocity of median nerve and ulnar nerve in both groups were higher than that before the treatment,but the comparison difference was insignificant(P>0.05),and the comparison difference between both groups was also insignificant(P>0.05);After the treatment,the sensory conduction velocity of the common peroneal nerve in both groups was higher than that before the treatment,and the improvement of the sensory conduction velocity of the common peroneal nerve in the observation group was better than that in the control group,which was statistically significant(P=0.046).After the treatment,the sensory conduction velocity of the tibial nerve in both groups was higher than that before treatment,and the difference in the treatment of the observation group was statistically significant(P<0.05).There was no significant difference in the treatment of the control group(P>0.05),and there was no significant difference in the comparison between each groups(P>0.05).Conclusion:Comparing with regular premixed insulin,reasonable usage of liraglutide in the treatment of type 2 diabetes mellitus patients with peripheral neuropathy declares a definite hypoglycemic effect.At the same time,the levels of serum inflammatory index and oxidative stress index can be improved,without increasing the incidence of adverse drug reactions.In some patients,improvement of the sensory conduction velocity of lower limb nerves(common peroneal nerve)can also be observed. |
