| Objective:To explore the clinical characteristics and risk factors of febrile neutropenia in childhood acute leukemia patient,to study the common infection site of febrile neutropenia in childhood acute leukemia patient and the characteristics of pathogenic bacteria in combination with septicemia,and to study the significance of PCT and CRP in the diagnosis of febrile neutropenia with septicemia in childhood acute leukemia patient.Methods:The retrospective study method was used to collect the clinical data of acute leukemia patients who were hospitalized in the pediatric hematology ward of our hospital from January 2017 to December 2019 for statistical analysis.Results:1.The incidence of febrile neutropenia in childhood acute leukemia patient is related to the type of disease diagnosis,the length of stay,the type of inpatient ward area,the duration of neutropenia,and the severity of neutropenia(P<0.05),regardless of gender and age(P>0.05).The length of stay,duration and severity of neutropenia are independent risk factors for febrile neutropenia during or after chemotherapy in childhood acute leukemia patient(P<0.05).2.The most common infection site of febrile neutropenia in childhood acute leukemia patient was respiratory tract infection,and the incidence of septicemia with septicemia was 13.3%.The most common infection is Gram-negative bacteria,accounting for 58.8%.The common gram-negative bacteria are Escherichia coli and Klebsiella pneumoniae,and the Gram-positive bacteria account for about 41.2%.The common gram-positive bacteria are Streptococcus mitis Oral Streptococcus,Staphylococcus epidermidis,etc.Among them,35.3% were resistant to antibiotics.The most common is ESBLs positive drug-resistant bacteria.3.The serum PCT value of febrile neutropenia in childhood acute leukemia patient with septicemia was significantly higher than blood culture negative patient,but there was no significant difference in CRP.Conclusion:1.The incidence of febrile neutropenia in childhood acute leukemia patient is related to the type of disease diagnosis,the length of stay,the type of inpatient ward area,the duration of neutropenia,and the severity of neutropenia,regardless of gender and age.The length of stay,duration and severity of neutropenia are independent risk factors for febrile neutropenia during or after chemotherapy in childhood acute leukemia patient.2.The common infection site of febrile neutropenia in childhood acute leukemia patient are respiratory tract,oral mucosa,gastrointestinal tract,blood flow,etc;Gram-negative bacteria are the most common pathogens of febrile neutropenia with septicemia,and the common pathogens of Gram-negative bacteria septicemia are Escherichia coli,Klebsiella pneumoniae,etc;The common Gram-positive bacteria septicemia is Streptococcus mitis Oral Streptococcus,Staphylococcus epidermidis,etc.3.The serum PCT value of febrile neutropenia in childhood acute leukemia patient with septicemia was significantly higher than blood culture negative patient(P<0.05),but there was no significant difference in CRP(P>0.05). |
