| Objectives To observe and analyze the effects of continuous blood purification on immune function and inflammatory factors in the treatment of severe sepsis in children.MethodsSelected 46 children with sepsis treated in Guangzhou Women and Children’s Medical Center from 2019 to 2021.There were 24 cases in continuous blood purification group(CBP group)and 22 cases in routine treatment group.Collected the general data,laboratory tests,immune function tests,inflammatory factor tests and other related results and analyzed its clinical characteristics.ResultsThere was no significant difference between continuous blood purification group and routine treatment group in gender,age,basic disease and primary infection site before treatment.Compared with the conventional treatment group,the continuous blood purification treatment group may have a positive protective effect on organ function(cardiovascular function,respiratory function and renal function)after treatment.Comparison of continuous blood purification therapy group with conventional therapy group in terms of immune function.Total white blood cell count,granulocyte count,lymphocyte count,T lymphocyte count,B lymphocyte count,NK cell count,C reactive protein(CRP),procalcitonin(PCT),interferon α(TNF-α)、 interferon β(TNF-β)、There were no significant differences in the levels of interleukin 6(IL-6),interleukin 8(IL-8),or interleukin 10(IL-10)at day 0(0d)(P > 0.05).Among them the specific inflammatory factor TNF-α、 IL-6,IL-8,and IL-10 showed a decreasing trend on day 1(1d),in the CBP group,with statistically significant differences(P <0.05)between the two groups.However,the nonspecific inflammatory parameters total leukocyte count(2d),granulocyte count(2d),lymphocyte count(2d),T lymphocyte count(1d),B lymphocyte count(2d),NK cell count(5d),CRP(1d),and PCT(2d)also showed statistical differences(P < 0.05).In which TNF-β 、 the neutrophil functions(PBS,PMA,SI)showed no statistical differences over the course of the study(P > 0.05).The PRISM III score showed a decreasing trend in outcome comparison between the two groups,statistical differences(P < 0.05)began to appear at 2d,PCIS all exhibited an increasing trend,statistical differences(P < 0.05)began to appear at 3d.Compare the 28 day survival rate,In CBP group,19 cases survived and 5cases died(79.1%),while in routine group,13 cases survived and 9 cases died(59.0%),There was significant difference between the two groups(P < 0.05).Conclusions1.Continuous blood purification therapy can regulate immune dysfunction in children with sepsis to some extent.2.Continuous blood purification therapy can effectively remove some inflammatory factors and improve the long-term survival rate in children with sepsis. |
