| Objectives: Musculoskeletal ultrasound(MSUS)was used to study the ultrasonic changes of gastrocnemius muscle in different courses of type 2diabetes(T2DM),and to explore the application value of musculoskeletal ultrasound in the evaluation of T2 DM gastrocnemius muscle lesions.Method: From December 2020 to January 2023,115 subjects who came to Guilin Traditional Chinese Medicine Hospital of Guangxi and met the inclusion and exclusion criteria were randomly selected.Among them,63 patients with T2 DM were used as the experimental group,including 33 patients with a course of less than or equal to 5 years as the experimental group 1,and 30 patients with a course of 5-13 years as the experimental group 2.52 cases of healthy people as normal control group.MSUS examination of the medial head of the right calf gastrocnemius muscle was performed on the two groups of subjects.The experiment used Mindray Resona 7 EXP ultrasonic diagnostic instrument,L14-5WU high frequency linear array probe and musculoskeletal mode.Observation methods and measurement indicators were as follows:(1)In the state of muscle relaxation,the integrity of the gastrocnemius muscle structure was observed.(2)Observe the ultrasound image of the right calf gastrocnemius muscle of the subject,and record the ultrasound image changes of whether the echo of the gastrocnemius muscle is enhanced,whether there is ’ ground glass ’like change,and whether there is fat infiltration.(3)The thickness(mm),fiber bundle length(mm),feather angle(°),shear wave velocity(m/s)and Young’s modulus(k Pa)of the medial head of the gastrocnemius muscle of the right leg were measured,and the results were recorded and saved.Statistical methods:SPSS 25.0 statistical software was used to analyze the experimental results.Results :(1)The incidence of three phenomena of gastrocnemius muscle echo enhancement,’ ground glass ’ like change and fat infiltration : the results were experimental group 2 > experimental group 1 > normal control group,the difference was statistically significant(P<0.05).(2)The experimental group 2was significantly lower than the normal control group,and the difference was statistically significant(P<0.05).(3)The measurement results of shear wave velocity(m/s)and Young ’s modulus(k Pa)showed that the values of the experimental group 1 were significantly lower than those of the normal control group,and the difference was statistically significant(P<0.05).The value of the experimental group 2 was significantly lower than that of the normal control group,and the difference was statistically significant(P<0.05).There was no significant difference between the experimental group 1 and the experimental group 2(P>0.05).(4)The measurement results of the thickness(mm)of the medial head of the gastrocnemius muscle showed that there was no significant difference between the experimental group 1,the experimental group 2 and the control group(P>0.05).(5)The measurement results of fiber bundle length(mm)showed that there was no significant difference between the experimental group1,the experimental group 2 and the control group(P>0.05).Conclusions :(1)Musculoskeletal ultrasound can be used for the diagnosis of diabetic myopathy.Diabetic myopathy can occur in the early stage of type 2diabetes and is positively correlated with the course of disease.(2)With the increase of the course of disease,the pinnate angle(°)of the gastrocnemius muscle in the middle and late stages of the disease will become smaller,and there is no significant change in the early stage.(3)Using SWE technology,it was found that the hardness of gastrocnemius muscle tissue in T2 DM patients decreased,which was not related to the course of disease.(4)With the increase of the course of disease,the thickness of the medial head of the gastrocnemius muscle(mm)and the length of the fiber bundle(mm)in T2 DM patients will not change. |
