Objective(s):This study investigates the basic information,knowledge of related medical policies and satisfaction with primary medical institutions of the elderly in the townships of Lingui District,analyzes the factors affecting the primary care of the elderly in the townships of xx District,analyzes the reasons affecting the medical behavior of the elderly,and provides a reference basis for further guiding the elderly in the townships of Lingui District to seek medical treatment in an orderly manner and promoting the construction of graded treatment and medical community.Method(s):Using a random stratified sampling method,elderly people in Lingui district townships were selected as the study sample,and a total of 12 administrative villages were selected in Liangjiang town,Sitang town and Liutang town,and 60 elderly people aged 60 and above were randomly selected from each administrative village,with a total of 720 people.The questionnaire was designed with reference to Anderson Health Service Utilization Model and combined with the field situation.The questionnaire included four aspects: basic information of the elderly,cognition of relevant medical policies,satisfaction with primary medical institutions,and medical consultation behavior.Valid questionnaires were collected and data were extracted for descriptive analysis,chi-square test,and binary logistic regression analysis using SPSS 26.0software.Result(s): A total of 720 questionnaires were distributed,and 665 valid questionnaires were collected:(1)Basic information of elderly people in Lingui district township:(1)mainly young elderly people(60-74 years old,71.28%),low education(elementary school education or below,82.71%),per capita monthly household income mostly from 2000-3500 yuan(70.89%),high participation rate(93.68%),and Most of the elderly could reach primary medical institutions within 10-20 minutes(69.62%).(2)Most elderly people self-assess their health(76.99%),but pay less attention to their health status(42.86%),and the number of participants is low(40.45%)under the national introduction of free medical checkups once a year for residents over 60 years old.(3)The overall prevalence of chronic diseases is high(47.52%),among which the prevalence of hypertension is the highest(31.13%)and 29.47% have multiple co-morbidities.(2)Knowledge of relevant medical policies:(1)The contracting rate of family doctors is low(33.53%).(2)The awareness of graded diagnosis and treatment and medical community is not high,49.92% and 32.18%respectively.(3)Satisfaction with primary medical institutions:(1)High satisfaction with doctors’ service attitude,medical environment and cost,89.02%,81.21% and 77.14%,respectively.(2)Satisfaction with doctors’ medical technology is low,not exceeding 60%.(4)Influencing factors of medical consultation behavior:(1)The elderly chose primary medical institutions for first consultation higher(77.03%),mainly because primary medical institutions are closer(98.33%),drugs can meet the needs of diseases(77.03%),and less concern about the medical technology of doctors(26.30%).(2)The results of one-way analysis of the factors influencing medical consultation behavior showed that there were statistical differences(P<0.05)in gender,age,marital status,education,income,occupation,type of insurance participation,knowledge of policies related to graded diagnosis and treatment and satisfaction with primary medical institutions among the elderly people for the first medical institution.(2)Binary logistic regression analysis found that elderly people who were female,had low income,low education,were jobless,had poorer self-rated health,and did not have regular medical checkups were more likely to choose primary medical institutions for first consultation.There was a positive relationship between the elderly who signed up with family doctors and those who had high knowledge of graded diagnosis and treatment and related medical policies.The higher the satisfaction with primary care institutions,the more the elderly tend to choose primary care.Conclusion :(1)Older people in Lingui Township have low education level,low economic level,and low understanding of relevant medical policies.(2)Most elderly people tend to go to primary care institutions for first consultation,among which those who are female,have low income,low education,are jobless,have poor self-assessed health,do not have regular medical checkups,and have higher knowledge of hierarchical medical treatment and related medical policies and satisfaction with primary care institutions are more likely to choose primary care for first consultation,with a positive relationship.(3)Therefore,the relevant departments need to take locally appropriate measures to strengthen the relevant medical policies and promote health education,improve the service capacity of primary medical institutions,and better promote the formation and development of a new medical care order and promote the health of the elderly.
