Objective:Recurrent urinary tract infection(rUTI)is relatively common clinically,but the current treatment and prevention effects are not ideal,which seriously affects the patient’s body and mind.In this study,by comparing and analyzing the differences in humoral immunity,cellular immune function indicators,nutrition and metabolism-related indicators of rUTI patients,patients with occasional urinary tract infection and normal healthy people,the immunology,nutrition and metabolism-related mechanisms of rUTI pathogenesis were explored.Provide evidence-based medical basis for the clinical treatment and prevention of rUTI.Methods:A total of 113 patients with uncomplicated urinary tract infection who met the inclusion and exclusion criteria and had complete clinical data in our hospital from October 2020 to December 2022 were collected.According to the frequency of infection,they were divided into sporadic urinary tract infection group(sporadic group,54 cases)and recurrent urinary tract infection group(recurrent group,59 cases),at the same time,54 healthy normal people who matched the age of the research subjects were selected to receive health examinations in our hospital during the same time period as the normal control group.A population of 54 cases served as the normal control group.Before the drug treatment,the humoral immunity,cellular immune function indexes,nutrition and metabolism related indexes of the two groups of patients with urinary tract infection were measured,and the above indexes of the healthy controls were measured by the same method,and the changes and differences of the above indicators between the two groups of patients and healthy people were compared and analyzed.Results:(1)Humoral immune indexes:There were significant differences in the levels of C3,C4,ceruloplasmin and transferrin among the three groups(all P<0.05).Among them,the level of C3 in the sporadic group was significantly higher than that of the recurrent group and the normal control group(both P<0.05);the level of C4 in the sporadic group and the recurrent group was significantly higher than that of the normal control group(both P<0.05);the level of ceruloplasmin in the occasional group was significantly higher than that in the recurrent group(P<0.05),and the level of ceruloplasmin in the sporadic group and the recurrent group was significantly higher than that in the normal control group(all P<0.05);there were no significant differences in the levels of IgG,IgA,IgM,IgE,κlight chain,λlight chain andκ/λamong the three groups(all P>0.05);(2)Cellular immune indicators:among the three groups There were significant differences in the levels of CD3~+T cells,CD4~+T cells and CD56~+T cells(all P<0.05).Among them,the level of CD3~+T cells in the recurrent group was significantly lower than that of the occasional group and the normal control group(both P<0.05);the level of CD4~+T cells in the sporadic group was significantly higher than that of the recurrent group and the normal control group(both P<0.05);the CD56~+T cell levels in the occasional group were significantly lower than those in the recurrent group and the normal control group(all P<0.05);there was no significant difference in the levels of CD8~+T cells,CD4/CD8,CD19~+T cells,and NK-like T cells among the three groups(all P>0.05);(3)Nutritional and metabolic indicators:there were significant differences among the three groups in albumin,globulin,white globulin ratio,urea,creatinine,sodium ions and calcium ions(all P<0.05).Among them,the albumin level in the normal control group was significantly higher,the globulin level was significantly lower,and the white globulin ratio was significantly higher than that in the occasional group and the recurrent group(both P<0.05);the urea level in the occasional group was significantly lower than that in the recurrent group and the normal control group(all P<0.05);the creatinine level of the normal control group was significantly higher than that of the recurrent group and the occasional group(both P<0.05),and the creatinine level of the occasional group was significantly lower than that of the recurrent group(P<0.05);and calcium ion levels were significantly higher than the occasional group and recurrent group(both P<0.05).Conclusion:Recurrent urinary tract infections are closely related to the body’s immune function and metabolic levels.In humoral immunity,recurrent episodes of urinary tract infections are mediated by a decrease in C3 and ceruloplasmin;In cellular immunity,recurrent urinary tract infections are closely related to the decline of CD3+T cells and CD4+T cells and the increase of CD56+T cells.In body metabolism,an increase in the body’s metabolites creatinine and urea can increase susceptibility to urinary tract infections.
