Objective:The aim of this study is to investigate the correlation between triglyceride glucose index(TyG),TyG combined with obesity indicators(waist circumference(WC),body mass index(BMI),waist-to-hip ratio(WHR),waist-to-height ratio(WHt R))and retinopathy(DR)in patients with type 2 diabetes mellitus(T2DM).The predictive value of TyG index and TyG combined with obesity indicators for DR Was analyzed.Materials and Methods:This study was a retrospective study.According to the inclusion and exclusion criteria,a total of 373 patients with T2 DM who were hospitalized in the Department of Endocrinology of Hebei General Hospital from September 2018 to May 2021 were recruited.According to the presence or absence of DR,the subjects were divided into T2 DM without DR Group(291 cases)and T2 DM with DR Group(192 cases).The clinical data of the patients were collected,and the required indicators TyG,TyG-WC,TyG-BMI,TyG-WHR,TyG-WHt R were calculated.Independent-sample t test was used to compare the difference of measurement data in accordance with normal distribution between groups.Mann-Whitney U test was used to compare the difference of non-normal distribution measurement data between the two groups,and chi-square test was used to compare the count data.Correlation analysis was performed using Spearman correlation analysis.Binary Logistic regression analysis was used for multivariate analysis.The receiver operating characteristic(ROC)curve was established to calculate the area under the curve(AUC),and the predictive value of TyG,TyG-WC,TyG-BMI,TyG-WHR and TyG-WHt R for DR Was analyzed.Result:1.The age,duration of diabetes,WC,WHR,WHt R,TG,SCr,BUN,FBG,Hb A1 c,TyG,TyG-WC,TyG-BMI,TyG-WHR and TyG-WHt R in T2 DM with DR Group were significantly higher than those in T2 DM without DR Group.GFR was significantly lower than that in T2 DM without DR Group(P < 0.05).2.Spearman correlation analysis showed that T2 DM with DR Was positively correlated with age,duration of diabetes,WC,WHR,WHt R,TG,SCr,BUN,FBG,Hb A1 c,TyG,TyG-WC,TyG-BMI,TyG-WHR and TyG-WHt R(P < 0.05).And negatively correlated with GFR(P < 0.05).3.Multivariate Logistic regression analysis showed that TyG,TyG-WC,TyG-BMI,TyG-WHR and TyG-WHt R were independent risk factors for DR In T2 DM patients after fully adjusting for confounding factors.4.ROC analysis showed that the AUC of TyG,TyG-WC,TyG-BMI,TyG-WHR and TyG-WHt R all had predictive value for T2 DM with DR,and the AUC of TyG-WHR(0.623)was the largest.The maximum Youden index was 0.205,and the best critical value of TyG-WHR was9.8027,and the corresponding sensitivity and specificity were 52.7% and67.8%.Conclusion:1.T2 DM with DR Was positively correlated with TyG,TyG-WC,TyG-BMI,TyG-WHR and TyG-WHt R.2.TyG,TyG-WC,TyG-BMI,TyG-WHR and TyG-WHt R were all independent risk factors for T2 DM with DR.3.TyG,TyG-WC,TyG-BMI,TyG-WHR and TyG-WHt R all have predictive value for DR In T2 DM patients,and TyG-WHR has the best performance,which is expected to become a new predictor for early identification of the risk of DR In T2 DM patients. |
