| Objective :Through observation and analysis of the short-term clinical efficacy and safety of point-matrix CO2 laser in the treatment of mild and moderate vaginal relaxation,to provide clinical reference for the treatment of patients with mild and moderate vaginal relaxation.Method :From September 2020 to September 2022,36 patients with mild or moderate vaginal relaxation were selected from the gynecological outpatient department of Guilin People’s Hospital who were treated with vaginal dot matrix CO2 laser therapy for a course of three times(with an interval of 28-45days).The self-reported symptoms and pelvic floor muscle strength assessment of all patients before and after treatment were recorded. At the same time, the questionnaire surveys of vaginal laxity questionnaire(VLQ)and female sexual function index(FSFI)were completed before and after treatment. The intraoperative and postoperative adverse reactions of patients were followed up and the results were analyzed.Result(s):(1)After dot-matrix CO2 laser treatment,28 of the 36 patients experienced moderate or above improvement of vaginal relaxation symptoms, and the effective rate was 77%(28/36).Among the 20 patients with stress urinary incontinence,16 patients showed significant improvement of urinary leakage symptoms after treatment, and 2 patients were cured clinically, with an effective rate of 80%(16/20).After laser treatment, one patient felt obvious vaginal itching and discomfort symptoms, which lasted for 3-4 days and gradually improved by themselves. The other patients did not complain special discomfort symptoms during and after surgery, and the incidence of adverse reactions was 2.7%.(2)After dot-matrix CO2 laser treatment, the scores of the vaginal relaxation questionnaire in patients improved. All scores and total scores in FSFI were significantly improved compared with those before treatment, with statistical significance(P < 0.05).(3)The mean value of Class I muscle fiber contraction was 24.67±8.62 before treatment, and 30.36±13.15 after treatment. The mean endurance contraction of pelvic floor muscle before treatment was 21.84±7.61,and that after treatment was 25.55±8.51,the difference was statistically significant(P < 0.05).The maximum value of Class II muscle fiber contraction was 36.16±19.04 before treatment and 41.96±17.44 after treatment, the difference was not statistically significant(P > 0.05).The contraction and relaxation time of class I and II pelvic floor muscle fibers was not significantly improved before and after treatment, and the difference was not statistically significant(P > 0.05).Conclusion(s):(1)dot-matrix CO2 laser demonstrates a significant effect on mild and moderate vaginal relaxation.(2)It can effectively improve the mean contraction value of Class I muscle fibers of pelvic floor and increase the endurance of pelvic floor muscle;(3)In the process of dot-matrix CO2 laser therapy, patients did not experience significant pain, short treatment time, simple operation,no obvious side effects, short postoperative recovery time, significant effect, worthy of clinical promotion. |
