Clinical Observation On Treatment Of Spleen Stomach Damp Heat Type Acne With Modified Xiehuang Powder Based On "Release Of Fire Stagnation"

Objective: The purpose of this study was to explore the effectiveness and safety of Modified Xiehuang Powder to treat damp-heat acne of spleen and stomach based on the theory of " Release of fire stagnation ",in order to provide theoretical basis and new effective methods for clinical treatment of this disease.Methods: A total of 92 patients with spleen-stomach damp-heat acne who met the inc lusion criteria in the Department of Dermatology,Shijiazhuang Hospital of Traditional Chinese Medic ine from January 2022 to October 2022 were selected as the research objects.They were randomly divided into treatment group and control group,with 46 cases in each group.The control group was treated with oral Qingreanchuang capsule combined with external fusidic acid cream.The treatment group was treated with Modified Xiehuang Powder combined with fus idic acid cream.Both groups were treated for 4 weeks,and the recurrence after 3 months was followed up.The changes of acne lesions(lesion type,quantity,shape,color,and degree),TCM syndrome scores,specific quality of life scores,and advers e reactions after treatment were recorded before and after treatment.Combined with the serum inflammatory factor IL-17 before and after treatment,the curative effect was evaluated according to the skin les ion score scale and TCM syndrome score scale.The data were analyzed by SPSS Statistics 22.0 software,and the efficacy and safety of Modified Xiehuang Powder in the treatment of acne with spleen and stomach dampness-heat were evaluated by comparing the difference of efficacy between the two groups.Results:(1)There was no significant difference in gender,age,lesion grade,course of disease and other basic conditions between the two groups before treatment(P(29)0.05),and the data of the two groups were comparable.(2)After treatment,the skin lesion scores of the two groups were lower than those before treatment,and the improvement of acne les ions in the treatment group was better than that in the control group(P(27)0.01).(3)In terms of TCM syndrome scores,the total scores of skin pain,skin redness,skin oil,bitter mouth and bad breath,urine and feces in the two groups were all reduced,and the treatment group had better results(P<0.01).(4)The total effective rate was 89.5% in the treatment group and71.0% in the control group(P<0.05),the difference between the two was statistically significant.(5)Both groups could reduce the serum level of IL-17(P<0.01),and there was no significant difference between the groups(P(29)0.05);(6)After 4 weeks of treatment,the QOL-Acne score of treatment group was lower than that of control group(P<0.05),the difference between the two was statistically significant.(7)After 3 months of follow-up,the recurrence rate of the treatment group was 7.1%,which was lower than 46.6% of the control group(P<0.05).During the treatment,no serious adverse events occurred.Conclusion: Based on " Release of fire stagnation ",Modified Xehuang Powder combined with fus idic ac id cream has good clinical efficacy in the treatment of acne with spleen and stomach damp-heat,which can improve the skin les ions of patients,relieve TCM syndromes,reduce the levels of inflammatory factors,improve the clinical treatment effect,control the recurrence of acne,and finally improve the quality of life,which is worthy of clinical research and reference.
