Clinical Observation Of A Repair Essence To Improve Skin Repair After CO2 Fractional Laser Surgery On Acne Atrophic Scar

Objectives:A variety of non-invasive skin detection techniques,such as multi-probe non-invasive skin detector(CK),reflectance confocal microscopy,dermatology and the Phaseshift Rapid In Vivo Measurement of the Skin(PRIMOS)were used to evaluate the effect of a repair essence containing Salvia miltiorrhiza(Danshen)Root extract and Porphyridium on the treatment of acne atrophic scars as well as skin barrier function after CO2laser surgery.Methods:The study was a single-blind,split-face controlled trial involving 31volunteers aged 20-30 years with acne atrophic scars of equal severity on both cheeks,based on inclusion and exclusion criteria.After CO2laser surgery,all patients were treated with a unified collagen cold compress patch for 30 minutes on their whole face.When the cold compress was removed,we applied the subject product to the right face and forearm,applied the control product to the left face and forearm in turn,and let the patients continue to use the product as required after returning home for 30days in a row.By applying the non-invasive skin testing equipment to objectively detect biophysical properties of the skin,subjective assessment was conducted by the physician using the ECCA acne scars scale,complications and adverse reactions during treatment and follow-up were observed and recorded,and patients were asked to score the satisfaction scale.Results:One month after treatment,the ECCA scores of the experimental group and the control group were significantly lower than those before treatment,and the scores of the experimental group were lower than those of the control group,the difference was statistically significant(P<0.05).PRIMOS indicated that 1 month after treatment,the sa and sq values on the experimental side were lower than those before treatment(P<0.05),and the sa and sq values on the experimental side were lower than those on the control side(P<0.05),which was consistent with clinical manifestations.The value of transepidermal water loss(TEWL)on both sides of cheeks increased obviously after laser treatment and recovered immediately after applying test and control products to the last follow-up.The TEWL recovery value at each time point on the experimental side was higher than that on the control side,and the difference was statistically significant(P<0.01).In normal forearm skin,there was no significant change in TEWL value within 1 week after using the product,but the difference of TEWL between the two groups increased at T6;The water content of stratum corneum decreased significantly and immediately after laser surgery(P<0.05),and then increased significantly to the peak value after icing for 30 minutes while using the product(P<0.05),and the water content of experimental side was significantly higher than that of control side at T4(P=0.019);In normal forearm skin,the water content of the stratum corneum at each time point of the experimental side was significantly increased compared with that at T0(P<0.05),but the water content of the control group at T6 had no statistical significance compared with that at T0(P>0.05).Immediately after CO2laser surgery under RCM,the blood flow in the superficial dermis was accelerated,the edema of the spinous cells was obvious,the collagen was not significantly changed immediately,and the epidermal thickness was thinner than that before treatment,but the difference was not statistically significant(P>0.05).During 2-7 days after surgery,new collagen appeared in the dermis,blood flow velocity was improved compared with before treatment,cell edema gradually subsided,epidermal thickness was thickened,and the recovery of inflammation in the right cheek was better than that in the left cheek.One month after surgery,the collagen on the right side was denser than that on the left side,and the epidermis on the right side was thicker than that on the left side,with statistical significance(P=0.039);There were no significant changes in the image and epidermal thickness of normal forearm skin under RCM after using the product;Although there was no significant difference in the skin morphology of both cheeks under dermoscopy,the process of skin barrier recovery before and after treatment could be observed;The patients’satisfaction with the overall effect,comfort,redness relief effect,itching relief effect and tingling relief effect of the test product was higher than that of the control product,and the average satisfaction of the right side was significantly higher than that of the left side(P=0.033).In addition,during the treatment period,no patients reported adverse reactions after the use of the test substance and control substance.The average time for the start of scab shedding and complete scab shedding on the experimental side were earlier than those in the control group.Conclusion(s):This repair essence has strong anti-inflammatory,repair and moisturizing effects,could effectively promote skin wound recovery after CO2fractional laser surgery.It improves patients’scar symptoms and skin barrier function.At the same time,it is safe.Long-term use also has some improvement on the barrier function of normal skin.
