Objective(s): Intra-knee fracture is a common fracture type in young adults with traffic accidents,and its surgical reduction and fixation directly affect the function of the joint after recovery.The purpose of this study is to explore the clinical efficacy of arthroscopic assisted reduction and internal fixation and traditional open reduction and internal fixation in the treatment of intra-knee fractures,and to provide a new idea and method for the treatment of intra-knee fractures.Methods: In this study,67 patients with intra-knee fractures(tibial plateau fractures and distal femoral fractures)admitted to our hospital from January 2018 to October 2022 were selected as the research objects.They were divided into ORIF group and ARIF group according to different surgical methods.Among them,there were 23 cases of tibial plateau fracture and 11 cases of distal femur fracture in ORIF group.In ARIF group,there were 21 cases of tibial plateau fracture and 12 cases of distal femur fracture.The two groups were statistically analyzed in terms of operation duration,incision size,intraoperative blood loss,hospitalization days,postoperative complications,HSS score and Lysholm score,and the difference was statistically significant(P<0.05).Results: Through retrospective analysis and statistics,the following results were obtained: the hospitalization days in ORIF group and ARIF group were(11.40 ± 2.27)d and(8.56 ± 4.39)d respectively;The operation time was(109.42 ± 33.75)min and(91.09 ± 39.15)min respectively.The incision size was(11.42 ± 4.17)cm and(8.09 ±4.09)cm,respectively.The intraoperative blood loss was(273.57 ± 72.07)ml and(175.87 ± 42.18)ml,respectively.Six months after operation,the HSS scores were(68.40 ± 5.94)and(79.15 ± 5.26)respectively.Lysholm scores were(70.71 ± 7.37)and(79.28 ± 5.35)respectively.12 months after operation,the HSS scores were(82.20 ± 9.93)and(91.18 ± 5.85)respectively.Lysholm scores were(85.88 ± 8.21)and(93.96 ± 4.19)respectively.Comparing the data of the above two groups,the ARIF group was superior to the ORIF group in terms of operation duration,incision size,intraoperative blood loss,hospitalization days,postoperative complications,HSS score and Lysholm score,and the difference was statistically significant(P<0.05).The excellent and good rates of HSS and Lysholm score in ORIF group were 78.71% and73.56% respectively.The excellent and good rates of HSS and Lysholm score in ARIF group were 85.53% and 86.25% respectively.There was no significant difference in postoperative complications between the two groups(P>0.05).Conclusion(s): Compared with ORIF group,ARIF group has the advantages of less trauma,shorter operation time,less intraoperative bleeding,better hospital stay and better postoperative knee joint function recovery.At the same time,the patients with meniscus injury and partial cruciate ligament injury(avulsion fracture at the anchorage)were evaluated early and treated in the first stage.
