ObjectiveIn this study,the clinical treatment of patients with Yang deficiency and water-panic coronary heart disease chronic heart failure was carried out by using Jia Wei Zhen Wu Tang,and its effects on the TCM symptoms,NT-pro BNP levels and 6-minute walking distance of the study subjects were observed,and the effects of Jia Wei Zhen Wu Tang on the cardiac function of the study subjects were evaluated from the perspective of hemodynamics,from which the efficacy and advantages of Jia Wei Zhen Wu Tang in treating Yang deficiency and water-panic coronary heart disease chronic heart failure were summarized to further expand the therapeutic ideas and practice of TCM for coronary heart disease chronic heart failure.MethodsFrom December 2021 to February 2023,this study collected patients with chronic heart failure caused by coronary artery disease(Yang vacancy and water pervasiveness syndrome)from the outpatient and inpatient wards of the Department of Cardiology,Guangzhou Hospital of Traditional Chinese Medicine,affiliated to Guangzhou University of Traditional Chinese Medicine,and included 62 patients who met the diagnostic criteria of traditional Chinese medicine and western medicine as the study subjects.The selected subjects were divided into treatment group and control group according to the principle of randomization.The control group was given standard western medicine treatment of coronary heart disease and chronic heart failure.The treatment group was treated with modified Zhenwu Decoction on the basis of standardized treatment of western medicine.After 4 weeks of treatment,the curative effects of traditional Chinese medicine and western medicine,TCM syndrome score,NYHA cardiac function grading(New York Heart Association cardiac function grading,see Appendix 3,the same below),walking distance(exercise tolerance),hemodynamic parameters such as cardiac output(CO)The changes of stroke volume(SV)and other indicators were recorded in detail.After the treatment,SPSS25.0 software was used for statistical analysis of professional data.Results1.Comparison of the TCM syndrome score and NYHA heart function classification before and after treatment: the total effective rate of improving the TCM syndrome score was 96.8% and 71.2% in the treatment group and the control group,respectively,and the treatment group was better than the control group(P < 0.05);the effective rate of improving the heart function in the treatment group and the control group was 96.8% and71.2%,respectively,and the treatment group was significantly better than the control group(P < 0.05).0.05)2.contrast of the plasma markers: the degree of NT-pro BNP decrease in both two group after treatment was significantly different(ΔP=0.004<0.05),and it can be regarded that the therapy group has better efficacy in lowering NT-pro BNP level than the other group.3.Comparison of exercise tolerance: there was no significant difference in the6-minute walking distance of the two groups before treatment,which was comparable,and the difference between the treatment group and the control group was statistically significant(P <0.05).Therefore,it can be considered that the curative effect of improving the walking distance in the treatment group was significantly better than that of the control group.4.Hemodynamic index of left ventricular output capacity: left ventricular ejection fraction(LVEF),cardiac output(CO),output(SV),comparing left ventricular ejection fraction(LVEF),cardiac output(CO),stroke output(SV),the difference was statistically significant(P <0.05),the effect was better than that of the control group.5.Heart preload and afterload: the early diastolic filling rate(EDFR)and the peripheral vascular resistance index(SVRi)of the two groups were comparable,and the early diastolic filling rate(EDFR)and the peripheral vascular resistance index(SVRi)were statistically significant(P <0.05).The treatment group was significantly better in reducing preload and afterload than that of the control group.ConclusionThe effect of flavored Zhenwu soup in the treatment of chronic heart failure is remarkable,Relative to the standardized western medicine treatment alone,Combined treatment with flavored Zhenwu soup has more advantages in improving the TCM clinical syndrome of chronic heart failure,improving the exercise tolerance,reducing the markers of heart failure,and improving the classification of heart function(NYHA heart function classification),And showed the consistency with the clinical efficacy in improving the efficacy of the noninvasive hemodynamic parameters,To prove the efficacy of treating chronic heart failure from clinical symptoms,activity tolerance and hemodynamics,The non-invasive hemodynamic measurement system is expected to be a new method to evaluate the efficacy of TCM in patients with chronic heart failure.
