| Objective:To evaluate the early analgesic effect and functional recovery of the hip joint after THA by applying cocktail therapy combined with postoperative analgesic pump.To provide a clinical basis for the analgesia of cocktail therapy combined with postoperative ketorolac aminotriol pain pump in total hip arthroplasty.Methods:Patients admitted to our hospital for the first unilateral THA between January 2021 and November 2022 were collected.Eighty-six patients were selected according to the inclusion criteria,and the patients were randomly divided into three groups according to the different analgesic regimens: group A was the combined treatment group,with 26 patients,who were given intraoperative periprosthetic cocktail injection and ketorolac aminotriol pain pump after returning to the ward;group B was the pain pump alone group,with 32 patients,who were treated with ketorolac aminotriol pain pump only after returning to the ward after surgery;group C was the cocktail alone group,with 28 patients,who were given intraoperative periprosthetic cocktail injection.Group C was the cocktail group alone,with 28 patients given intraoperative cocktail injection around the hip joint.The duration of hospitalization in the three groups was analyzed and compared,and the VAS scores and Harris hip function scores were calculated for the three groups at rest and in the active state after surgery.The postoperative blood loss and the first time to the ground were observed in the three groups.Results:In the comparison of the length of stay of the three groups of patients,group A was smaller than groups B and C.The difference was statistically significant at P<0.05.The VAS scores in the resting state at 12 h,24h,48 h and 72 h after THA in the three groups were lower in group A than in groups B and C.The difference was statistically significant.In the VAS scores in the active state,the scores in group A were lower than those in groups B and C,P<0.05,and the difference was statistically significant.The results of Harris hip function scores under active status showed that the Harris hip function scores in the three groups were higher in group A than in groups B and C at 12 h,24h and 48 h after surgery,with P<0.05,and the difference was statistically significant.The comparison of blood loss after THA in the three groups of patients was lower in groups A and C than in group B.The difference was statistically significant at P<0.05.The results of the first postoperative time to the ground showed that the first time to the ground in group A was earlier than that in groups B and C.The difference was statistically significant at P<0.05.Conclusion:Application of cocktail therapy combined with postoperative ketorolac aminotriol analgesic pump can significantly reduce early postoperative pain,improve hip function,early rehabilitation exercise and reduce hospitalization time in THA patients.At the same time,the cocktail with tranexamic acid can significantly reduce postoperative bleeding in THA and help the timely recovery of hip function,which is a safe and effective analgesic modality with high practical value and can be widely used in clinical practice. |
