Construction And Verification Of Post Competency Ev Aluation Index For Digestive Endoscope Cleaning And Disinfection Personnel

Objective: Based on the competency iceberg model,through literature review,qualitative interview,expert consultation and cross-sectional survey,this study aims to build a relatively systematic set of competency evaluation indicators for digestive endoscope cleaning and disinfection personnel,to standardize the evaluation of digestive endoscope cleaning and disinfection personnel in China,and to improve the access conditions and assessment system for digestive endoscope cleaning and disinfection personnel in China,so as to strengthen the management of digestive endoscope cleaning and disinfection personnel.Methods: The first part: preliminarily construct the evaluation index of post competency of digestive endoscope cleaning and disinfection personnel.On the basis of literature review,the objective sampling method was adopted.From January 2022 to March 2022,5 nursing managers of digestive endoscope,3 infection management experts and 7 cleaning and disinfection personnel of digestive endoscope were selected for semi-structured interview.The data analysis was carried out by using the Colaizzi7-step analysis method to extract the competency elements of the cleaning and disinfection personnel of digestive endoscope and form the first draft of the competency evaluation index of the cleaning and disinfection personnel of digestive endoscope.The second part: to establish the evaluation index of post competence of digestive endoscope cleaning and disinfection personnel.Based on the competency iceberg model,literature review and qualitative interview results,the evaluation index pool of post competency of digestive endoscope cleaning and disinfection personnel was developed and the expert consultation questionnaire was developed;The Delphi method was used to select 15 experts from different provinces in the field of nursing management and infection management.After two rounds of expert consultation,the evaluation indicators were screened and revised to establish the evaluation indicators for the post competency of digestive endoscope cleaning and disinfection personnel.The third part: Verification of the evaluation indicators of the post competency of digestive endoscope cleaning and disinfection personnel.Based on the evaluation index of the post competency of the digestive endoscope cleaning and disinfection personnel,the relevant items were refined to develop the evaluation table of the post competency of the digestive endoscope cleaning and disinfection personnel.The convenient sampling method was used to conduct the survey from August to November 2022,and338 valid questionnaires were collected for screening and reliability and validity test of evaluation index items.Results: The first part: Through literature review and analysis of interview data,the research team summarized the post competency elements of digestive endoscope cleaning and disinfection personnel into four themes: theoretical knowledge,operational skills,comprehensive ability,personal characteristics and motivation;13sub-themes: knowledge related to endoscope,knowledge related to endoscope cleaning and disinfection,knowledge related to infection control,work preparation,cleaning of endoscope and accessories,maintenance of working environment,critical thinking ability,emergency response ability,teaching ability,team cooperation and communication ability,etc.The second part: the effective recovery rate of the two rounds of expert consultation questionnaires is 100%,the rate of putting forward opinions is 73.33% and 40.00%respectively,the expert authority coefficient is 0.95 and 0.96 respectively,the Kendall’s W coefficient is 0.240 and 0.265 respectively(P<0.01),and the index variation coefficient of the second round of expert consultation is 0.00-0.22;The concentration of expert opinions is 3.80-5.00.The post competency evaluation indicators of digestive endoscope cleaning and disinfection personnel are constructed,including 4 first-level indicators,12 second-level indicators and 52 third-level indicators.The third part: based on the evaluation index of the post competency of the digestive endoscope cleaning and disinfection personnel,the relevant items were refined and the evaluation table of the post competency of the digestive endoscope cleaning and disinfection personnel was developed,with a total of 57 items.In this study,the critical ratio of each index item is 11.046-26.599(P<0.001);The I-CVI,S-CVI/UA and SCVI/Ave of the evaluation indicators are 0.833-1.000,0.769 and 0.961 respectively;A total of 6 common factors(secondary indicators)were extracted through exploratory factor analysis,with cumulative variance contribution rates above 60%;Delete three unclear tertiary indicator entries,and ultimately form a competency evaluation index for digestive endoscope cleaning and disinfection personnel that includes four primary indicators,six secondary indicators,and 54 tertiary indicators.The total Cronbach’s αcoefficient of the evaluation in system was 0.986;The overall correlation coefficient between the first-level indicators and indicators is 0.822-0.947,P<0.001.The result shows that the internal quality of the evaluation index is good.Conclusion: Through theoretical research,qualitative interviews,expert consultation,and verification of the indicators,this study constructs a post competency evaluation index for digestive endoscope cleaning and disinfection personnel,with a reasonable structure and good internal quality.The competency evaluation indicators for digestive endoscope cleaning and disinfection personnel can help endoscope cleaning and disinfection personnel clarify their own competency level and improve their endoscopic reprocessing ability.At the same time,they can also provide reference for human resource management such as admission and evaluation of endoscope cleaning and disinfection personnel in hospitals.
