| Objective(s):To compare the diagnostic value of transrecta l three-dimensional ultrasonography and pelvic magnetic resonance in the diagnosis of cervical cancer parametrial invasion,vaginal fornix invasion,lower uterine body invasion,rectal invasion,and bladder invasion,and then evaluate the3D-ERUS for local cervical cancer.Applied value of staging.Methods:This study retrospectively collected 111 patients with cervical cancer who underwent transrectal three-dimensional ultrasonography and pelvic magnetic resonance examination,surgery and pathological res ults at Yunnan Cancer Hospital from May 2021 to May 2022,the gold standard Postoperative histopathological results.Extract the imaging features of cervical cancer local staging by transrectal three-dimensional ultrasound and pelvic magnetic resonance: wh ether there is invasion of the parametrium,whether the vaginal vault is invaded,whether the lower uterine body is invaded,whether the bladder is invaded,whether the rectum is invaded,Comparative study of imaging evaluation results and postoperative pathological results.To evaluate the consistency between the local staging results of transrectal three-dimensional ultrasound and pelvic magnetic resonance and pathological results,the area under the curve(AUC),sensitivity(Se),specificity(Sp),positi ve predictive value(PPV),negative predictive value(NPV)accuracy(Acc).Results:1.Compared with the postoperative pathological results,the evaluation results of three-dimensional transrectal ultrasonography for cervical cancer with or without paramet rial invasion were as follows 99.09%,Sp 0%,PPV99.09%,NPV 0%,Acc 98.20%;The evaluation results of pelvic MRI for cervical cancer with or without parametrial invasion are as follows: Se99.09%,Sp 0%,PPV 99.09%,NPV 0%,Acc 98.20 %.2.Compared with the postoper ative pathological results,the evaluation results of three-dimensional transrectal ultrasonography for cervical cancer with vaginal vault invasion are as follows: Se 99.02%,Sp11.11%,PPV92.66%,NPV 50%,Acc 90.99%,AUC 0.630,The results of pelvic magnetic resonance assessment for cervical cancer with or without vaginal vault invasion are as follows: Se 99.02%,Sp 11.11%,PPV 92.66%,NPV 50%,Acc90.99%,AUC 0.630.The results of transrectal three-dimensional ultrasound and pelvic magnetic resonance for cervical can cer with or without vaginal vault invasion were significantly different from the pathological results(P<0.05).3.Compared with the postoperative pathological results,the evaluation results of the three-dimensional transrectal ultrasonography on whether th e cervical cancer has invasion of the lower uterine body and the pathological results are as follows: Se 91.74%,Sp 0%,PPV 98.04%,NPV 0%,Acc90.09%.The results of pelvic magnetic resonance in evaluating whether cervical cancer has invasion of the lower ute rine body are: Se 92.73%,Sp100%,PPV 100%,NPV 11.11%,Acc 91.89%.4.Compared with the postoperative pathological results,the evaluation results of three-dimensional transrectal ultrasonography for cervical cancer with or without bladder invasion were: Se 100%,Sp 0%,PPV 100%,NPV0%,Acc 100%。 The assessment of cervical cancer with or without bladder invasion is: Se 100%,Sp 0%,PPV 100%,NPV 0%,Acc 100%.5.Compared with the postoperative pathological results,the evaluation results of three-dimensional transrectal ultrasonography for cervical cancer with or without rectal invasion were: Se 100%,Sp 0%,PPV 100%,NPV0%,Acc 100%.The results of MRI for the assessment of cervical cancer with or without rectal invasion are Se 100%,Sp 0%,PPV 99.10%,NPV0%,Acc 99%.Conclusion(s):Three-dimensional transrectal ultrasonography for local staging of cervical cancer: the evaluation accuracy of parametrial invasion,vaginal vault invasion,lower uterine body invasion,rectal invasion,and bladder invasion is relatively high,and it is consistent with pelvic cavity invasion.The results of magnetic resonance assessment are similar,and three-dimensional transrectal ultrasonography can assist pelvic magnetic resonance to assess the local stage of cervical cancer... |
