Objective To investigate the clinicopathological features of patients with non-cardiac gastric adenocarcinoma in Ningxia and analyze the possible factors affecting their prognosis. Methods From January 2017 to December 2019,317 patients with non-cardiac gastric adenocarcinoma who underwent radical gastrectomy in Ningxia medical university general hospital were selected as the research objects.According to the location of primary tumor, they were divided into gastric body group and gastric antrum pylorus group, the Basic data and clinicopathological data were collected. The collected data were analyzed retrospectively, and the survival situation of patients was known through outpatient records,SMS and telephone.SPSS 26.0 was used for statistical analysis. Results A total of 317 patients with non-cardiac gastric adenocarcinoma were enrolled in this study, including 117 cases(37%)in the gastric body group and 200 cases(63%)in the gastric antrum pylorus group. There were 238(75.1%)males,79(24.9%)females,202(63.7%)poorly differentiated patients,90(28.4)moderately differentiated patients and 25(7.9%)highly differentiated patients.1.In terms of tumor size, the proportion of tumor size ≥ 5cm in gastric body group was higher than that in gastric antrum pylorus group, and the difference between the two groups was statistically significant(P < 0.05).2.According to Borrmann classification of advanced gastric cancer, the proportion of type III/ IV in gastric body group was higher than that in gastric antrum pylorus group, and the difference between the two groups was statistically significant(P < 0.001).3.There was no significant difference in age, sex, smoking, drinking, family history of stomach cancer,ABO blood type, differentiation degree,Hp infection,Lauren classification, lymph node metastasis, clinical stage,T stage and N stage between gastric body group and gastric antrum pylorus group(P > 0.05).4.Prognostic analysis: The overall survival rates were 64.8% in gastric body group and76.2% in gastric antrum pylorus group, and the mean survival time of the former was52.99±2.55 months, the latter was 58.53±1.70 months. There was significant difference in survival analysis between the two groups(P<0.05).5.Univariate analysis showed that age, location, tumor size, clinical stage,T stage and N stage were the risk factors affecting the prognosis of patients with non-cardiac gastric adenocarcinoma, and the differences were statistically significant(P < 0.05).6.Multivariate analysis showed that age,T stage and N stage were independent risk factors for prognosis of patients with non-cardiac gastric adenocarcinoma(P < 0.05).Conclusions 1.Males are more common in non-cardiac gastric adenocarcinoma, especially pyloric adenocarcinoma of gastric antrum, and the proportion of poorly differentiated gastric adenocarcinoma is high.2.Among non-cardiac gastric adenocarcinoma, gastric body adenocarcinoma has the clinical characteristics of poor Borrmann classification, and its prognosis is worse than that of gastric antrum pyloric adenocarcinoma.3.Age,depth of invasion and lymph node metastasis were independent risk factors for prognosis of patients with non-cardiac gastric adenocarcinoma.
