| Objective:Based on the“theory of meridional flow”,this study aims to explore the clinical effect of pressing beans on ear acupoints at the time of Ziwu Liuzhu on insomnia in patients with Qi and Yin deficiency type diabetic patients,give full play to the advantages and characteristics of traditional Chinese medicine non-drug therapy,and provide effective basis for improving the efficacy of auricular point pressing technique in diabetes mellitus with insomnia.Methods:A randomized controlled clinical study was conducted to select60 patients who met the inclusion criteria and were diagnosed with qi-Yin deficiency and insomnia from April 2018 to October 2018 in the endocrinology ward of a grade A hospital in Guangdong Province.A total of 60 patients signed informed consent to join the study.Randomly divided into observation group group and control group with 30 people each.On the basis of the control group,the observation group implemented Ziwu Liuzhu to press beans at selected ear points,and the control group received routine treatment and nursing care,including routine treatment of diabetes(lowering blood sugar,promoting blood circulation and removing blood stasis,lowering lipids,etc.)and daily nursing.The PSQI,clinical efficacy,TCM syndrome score,fasting blood glucose,postprandial blood glucose and blood glucose before bedtime were compared between the two groups before intervention(T0),7 days(T1)and 2 weeks(T2)after intervention.The count data were expressed as percentage,χ~2test and Kruskal-Wallis rank sum test were used.The measurement data were expressed as mean±standard deviation.Single factor analysis of variance,repeated measurement analysis of variance and LSD multiple analysis were used for normal distribution and homogeneity of variance.Kruskal-wallis test in non-parametric test was used for homogeneity of variance.P<0.05 was considered statistically significant.Results:1.Comparison of general baseline dataAfter study and analysis,there was no statistical significance in gender,age,education level,type of medical insurance,family history of diabetes,diet preference,course of diabetes,diabetic complications and glycosylated hemoglobin between the two groups(P>0.05),indicating that the two groups were comparable.2.Comparison of baseline data of clinical efficacy indicatorsThere was no significant difference in PSQI score and TCM syndrome score between the two groups before intervention(P>0.05),indicating that the two groups were comparable.3.PSQI scorePSQI scores in both groups decreased after intervention compared with before intervention(P<0.05),and the decline trend in observation group was better than that in control group,the difference was statistically significant(P<0.05).Independent sample T test was used for comparison between groups,and the results showed that PSQI scores of the observation group were lower than those of the control group at 7 and14 days after intervention,with statistically significant differences(P<0.05).4.Clinical curative effectUsing Pearson 2 test for statistical analysis,the total effective rate of TCM symptom curative effect in the observation group(96.67%)after 14days of intervention was higher than that of the control group(50.00%),and the difference was statistically significant(P<0.05).5.TCM syndrome scoreIntra-group comparison:TCM syndrome scores of patients in both groups decreased after intervention compared with before intervention,with statistically significant difference(P<0.01).Inter-group comparison:Independent sample T test was used,and the results showed that the TCM syndrome scores of the observation group were lower than those of the control group at 7 and 14 days after intervention,with statistically significant differences(P<0.05).6.Fasting blood glucoseFasting blood glucose in both groups decreased after intervention compared with before intervention(P<0.05),and the observation group had a better downward trend than the control group,the difference was statistically significant(P<0.05).Independent sample T test was used for comparison between groups,and the results showed that fasting blood glucose in the observation group was lower than that in the control group at 7 and 14 days after intervention,with statistically significant differences(P<0.05).7.Postprandial blood glucoseAfter intervention,blood glucose in both groups decreased compared with that before intervention and after meal(P<0.05),and the downward trend in observation group was better than that in control group,with statistical significance(P<0.05).Independent sample T test was used for comparison between groups,and the results showed that the postprandial blood glucose of the observation group was lower than that of the control group at 7 and 14 days after intervention,with statistically significant differences(P<0.05).8.Blood glucose before bedtimeBlood glucose in both groups decreased after intervention compared with before intervention(P<0.05),and the observation group had a better downward trend than the control group,the difference was statistically significant(P<0.05).The independent sample T test was used for the comparison between groups,and the results showed that the blood glucose of the observation group was lower than that of the control group at 7 and 14 days after intervention,with statistically significant differences(P<0.05).Conclusions:1.Pressing beans at auricular acupoints at the time of Ziwu Liuzhu injection is simple and easy,with significant effect,no clinical side effects,and easy acceptance and cooperation by patients.2.Meridian flow injection can effectively reduce patients’PSQI and improve their sleep quality.Reduce the TCM symptom score of patients,improve the total effective rate of treatment of patients with thirst and insomnia of qi-Yin deficiency type;The patients’fasting blood glucose,postprandial blood glucose and blood glucose before sleep were decreased,and their sleep quality was improved.Longer observational studies are needed to delay the occurrence of complications. |
