Objective: To observe the effect of Wenyang Jiedu Huayu Recipe on peripheral blood liver function,endotoxin and expression of M1/M2 macrophage-related cytokines in liver tissue of rats with acute-on-chronic liver failure(ACLF).To evaluate the therapeutic effect of Wenyang Jiedu Huayu Recipe on ACLF,and to explore its possible mechanism of anti-liver failure.Methods: 60 SD male rats were randomly divided into normal group,model group and traditional Chinese medicine group(Wenyang Jiedu Huayu Recipe).The rat model of immune hepatic fibrosis was established by bovine serum albumin sensitization method,and then the rat model of ACLF was established by acute intraperitoneal injection of D-Gal and LPS,and corresponding interventions were given respectively.The liver function,endo-toxin,morphological and pathological changes of liver tissue,and the expression of M1/M2 macrophages in each group were observed 1h,12 h,and 24 h after acute challenge.Results:1.Liver function: Compared with the normal group,ALT and AST in the model group were significantly increased(P<0.01),and compared with the model group,ALT and AST in the traditional Chinese medicine group were significantly decreased(P<0.01).2.Endotoxin: Compared with the normal group,the endotoxin in the model group was significantly increased(P<0.01),Compared with the model group,the endotoxin level in the traditional Chinese medicine group was significantly decreased(P<0.01).3.Liver histopathology and score: Compared with the normal group,the degree of liver tissue pathological damage in the model group was significantly increased(P <0.01).4.Detection of M1 and M2 macrophage markers in liver tissue by immunohistochemical method: Compared with the normal group,the expressions of CD68,i NOS,Arg-1 and CD206 in the model group were significantly increased(P <0.01).Compared with the model group,the expression of CD68 and i NOS in the traditional Chinese medicine group decreased at each time point(P <0.01,P <0.05),and the expression of i NOS decreased significantly(P <0.01);on the contrary,in the expression of Arg-1 and CD206,The Chinese medicine group increased at each time point(P <0.01,P <0.05).5.RT-qPCR assay for m RNA expression of M1 and M2 macrophage markers in liver tissue: Compared with the normal group,the expressions of CD68,i NOS,Arg-1 and CD206 in the model group were all up-regulated at 1h,12 h and 24h(P <0.01).Compared with the model group,the expression of i NOS in the traditional Chinese medicine group was significantly down-regulated at 1 h,12 h and 24 h(P <0.05),and the expression of CD68 was down-regulated at each time point(P <0.05).The expression of CD206 was up-regulated at 1h,12 h and24h(P <0.01,P <0.05),and the expression of Arg-1 was significantly increased(P <0.01).6.The ratio of M1 and M2 macrophage-related markers in liver tissue:compared with the blank group,the ratio of CD68/CD206 and i Nos/Arg-1 in the model group increased(P<0.01,P <0.05),and the polarization of M1/M2 shifted to M1;compared with the model group,CD68 /CD206,i Nos/Arg-1 ratios were down-regulated(P<0.01,P <0.05),and M1/M2 polarization shifted to M2.Conclusion: There is an imbalance of M1/M2 macrophage expression in ACLF rats;Wenyang Jiedu Huayu Recipe can improve liver tissue damage in ACLF rats,and its mechanism may be related to up-regulating CD206 and Arg-1,inhibiting the expression of CD68 and i NOS,and inducing macrophages.Phagocytosis is involved in the polarization from M1 to M2.
