| Purpose:In this study, ultrasonic conductance instrument was used to stimulate the acupoints around the knee joint, observe the relief of pain and swelling of the knee joint and the recovery of normal physiological function of the affected limb after arthroscopic anterior cruciate ligament reconstruction, and evaluate the clinical efficacy of ultrasound-guided drug acupoint penetration combined with conventional rehabilitation in alleviating symptoms such as swelling and pain of the knee joint and limited activity,To provide a certain clinical basis for the rehabilitation treatment of patients with anterior cruciate ligament reconstruction.Material and method:The inclusion and exclusion were carried out in strict accordance with the criteria.60 patients with unilateral anterior cruciate ligament injury of grade III and above who underwent surgical treatment in the second Department of orthopedics of the Affiliated Hospital of Liaoning University of traditional Chinese medicine from January 2020 to January 2022 were selected. All patients signed informed consent(see attached figure 1).All patients underwent anterior cruciate ligament reconstruction with autologous tendon under knee arthroscopy under general anesthesia, which was operated by the same operator. After operation, according to the method of doll’s clinical case random table(see attached Fig.2),they were randomly divided into control group and experimental group: 30 cases in the control group,16 males and 14 females, aged 20-60 years, with an average age of 40.50 years; There were 30 cases in the experimental group, including 17 males and 13 females, aged from 20 to 60 years, with an average age of 39.87 years. The two groups were treated with CPM exercise therapy, and the researchers guided routine functional exercise. On the second day after operation,on the basis of routine treatment, the experimental group received targeted acupoint penetration treatment with ultrasonic conductivity instrument on the affected side. The visual analogue(VAS)pain score, the degree of knee swelling and the passive flexion angle of the affected side of the two groups were counted and observed before operation, the second day after operation, the seventh day after operation, the 14 th day after operation and one month after operation.Using spss26 0 statistical software will collect and analyze the data of patients who have completed all courses of treatment and all evaluations according to the standard, and draw a conclusion.Results:1 There was no significant difference between the two groups in age, gender, weight and duration of disease(P > 0.05).2 On the second day after operation, there was no significant difference in visual analog VAS pain score, degree of knee swelling and passive motion angle between the two groups(P >0.05).3 one week, two weeks and one month after operation, the visual analogue(VAS)pain score, the degree of swelling and the angle of passive movement of the affected knee were compared between the two groups, and the differences were statistically significant(P < 0.05);4 one week, two weeks and one month after operation, the visual analogue(VAS)pain score, swelling degree and passive movement angle of the affected knee were compared between the two groups, and the differences were statistically significant(P < 0.05);5 There were no wound infection, wound nonunion and autologous graft immune rejection in the two groups within 1 month after operation.Conclusion:1.The surgical treatment of anterior cruciate ligament reconstruction under knee arthroscopy can effectively solve the symptoms of instability and dislocation of knee joint, restrict the abnormal activity between bone and bone, and use autologous tendon for ligament reconstruction without the risk of immune rejection.2.Postoperative combined with routine treatment can also improve the early postoperative symptoms.3.The application of ultrasonic conductance acupoint directional drug penetration therapy combined with the rehabilitation treatment after arthroscopic reconstruction of anterior cruciate ligament injury effectively alleviated the symptoms of postoperative pain and swelling,so that patients can carry out knee function training earlier, which is very helpful for the early recovery of normal physiological function of knee joint, and effectively prevented muscle atrophy and joint soft tissue adhesion. |
