Objective:To study the efficacy and safety of thin silver needle heat conduction combined with ozone injection and nerve root pulsed radiofrequency in the treatment of postherpetic neuralgia.Methods:Patients with postherpetic neuralgia who were admitted to China-Japan Friendship Hospital from March 2018 to May 2020 were divided into thin silver needle heat conduction combined with ozone injection group(group S)and nerve root pulsed radio frequency(group R).The baseline information and surgical data of the patients during hospitalization were collected,while the overall curative effect,NRS,AIS score,QL-index score and adverse events on the day of operation,3 months after surgery and 6 months after surgery were collected by telephone during follow-up.SPSS 25.0 statistical software was used for propensity score matching,and the interference of confounding factors in the baseline data was excluded.The caliper value was set to 0.02,and the matching was performed according to the ratio of 1:1.The results of the matched patients were analyzed.The NRS score,AIS score,QL-index score,and the overall curative effect were compared between the two groups before surgery,on the day of surgery,3 months after surgery,and six months after surgery.Results:There were significant differences in age and disease duration in the baseline data of the two groups,while the baseline information was balanced between the two groups by PSM.The postoperative NRS scores and AIS scores of the two groups were lower than those before surgery,and the QL-index scores were higher than those before surgery.There was no significant difference in the NRS score and AIS score on the day of surgery between the two groups,but there were statistical differences in the NRS score and AIS score at 3 months after surgery and 6 months after surgery,and the NRS score and AIS score in group S were lower in group R(P<0.05).QL-index scores on the day of surgery,3 months after surgery,and 6 months after surgery between the two groups are not different significantly.There was no significant difference in the effective rate on the day of surgery,3 months after surgery,and 6 months after surgery between the two groups.There was no significant difference in the recovery rate between the two groups on the day of operation and 3 months after the operation,and the recovery rate of the S group was higher than that of the R group at 6 months after the operation(P<0.05).There was no significant difference in the recovery rate of patients with neck,shoulder,upper limbs,waist and abdomen between the two groups on the day of surgery,3 months after surgery,and 6 months after surgery.There was no significant difference in the recovery rate of the chest and back between the two groups on the day of operation and 3 months after the operation.Six months after the operation,the recovery rate of patients in the S group was higher in the chest and back,and the difference was statistically significant(P<0.05).There was no statistical difference in the recovery rate of the patients in the long course group on the day of surgery,3 months after surgery,and 6 months after surgery between the two groups.There was no significant difference in the recovery rate of patients with a short course of disease on the day of surgery and 3 months after surgery between the two groups.The recovery rate of patients with short course of disease 6 months after operation in group S was higher(P<0.05).Adverse reactions were reported in one patient each in groups S and R.Conclusion:Thin silver needle heat conduction combined with ozone injection and nerve root PRF can treat PHN with good safety and efficacy,and both can alleviate the pain and improve sleep and quality of life.Silver needle combined with ozone injection improves pain and sleep better at 3 months and 6 months after operation.The recovery rate of thin silver needle heat conduction combined with ozone injection is higher,and it may be more advantageous for the treatment of patients with short duration and disease location in the chest and back. |
