ObjectivesTo observe the improvement of anxiety and depression in patients with chronic heart failure accompanied by anxiety and depression by abdominal acupuncture therapy,with a view to providing a new treatment method for traditional Chinese medicine to intervene in dual heart disease.Methods1.Select 66 patients with chronic heart failure and anxiety and depression who meet the criteria for inclusion,and randomly divide them into abdominal acupuncture group(33 cases)and control group(33 cases).2.The control group was treated with conventional anti heart failure drugs for 2 weeks.The abdominal acupuncture group was treated with abdominal acupuncture on the basis of conventional anti heart failure drugs,and abdominal acupuncture was performed 5 times a week,with 30 minutes of needle retention each time,for a total of 2 weeks of intervention.3.The observation indicators were the Generalized Anxiety Symptom Scale-7(GAD-7)score,the 9-item Patient Health Questionnaire(PHQ-9)score,the TCM symptom grading quantitative score,the Pittsburgh Sleep Quality Index(PSQI)score,and the Kansas City Cardiomyopathy Questionnaire(KCCQ-12)score,which were evaluated before and after treatment.4.Calculate the total effective rate and evaluate the efficacy through the reduction rate of GAD-7,PHQ-9,and TCM symptom grading quantitative scores.5.Record the safety indicators and adverse reactions during the research process,and evaluate the safety of abdominal acupuncture treatment.Results1.This study included 66 patients with chronic heart failure and anxiety and depression who met the inclusion criteria of this study and were hospitalized in the Department of Cardiology of Tongzhou Hospital of Dongzhimen Hospital from October 2022 to March 2023.In the abdominal acupuncture group,33 patients were enrolled,2 patients fell off,1 patient was excluded,and 30 patients were actually completed;In the control group,33 cases were enrolled,1 case fell off,1 case was removed,and 1 case was terminated.30 cases were actually completed.2.The baseline data are comparable:before treatment,the baseline data of age,gender,smoking history,heart failure course,heart function classification,heart failure type,concomitant diseases(coronary heart disease,hypertension,atrial fibrillation,diabetes,hyperlipidemia),anxiety and depression of the abdominal acupuncture group and the control group were statistically analyzed,and the P value was greater than 0.05,with no statistically significant difference,suggesting that the groups were comparable.3.The efficacy of abdominal acupuncture group in improving anxiety and depression of patients is better than that of the control group:Before treatment,the GAD-7 scores of the two groups are comparable.After treatment,the GAD-7 scores in both groups decreased compared to those before treatment,with a statistically significant difference within the group(P<0.001);Compared between groups,the GAD-7 score in the abdominal acupuncture group after treatment was lower than that in the control group(P<0.001);The total effective rate in the abdominal acupuncture group was 92.31%,while the total effective rate in the control group was 68.00%(P<0.05).It is suggested that the abdominal acupuncture group is superior to the control group in improving the anxiety state of patients.Before treatment,the PHQ-9 scores of the two groups were comparable.After treatment,the PHQ-9 scores in both groups decreased compared to those before treatment,and there was statistically significant difference between the two groups(P<0.001);Compared between groups,the PHQ-9 score of the abdominal acupuncture group after treatment was lower than that of the control group(P<0.001);The total effective rate in the abdominal acupuncture group was 89.29%,while the total effective rate in the control group was 62.96%(P<0.05).It is suggested that the abdominal acupuncture group is superior to the control group in improving the depressive state of patients.4.Abdominal acupuncture group is superior to the control group in improving TCM symptoms:Before treatment,the total and various scores of TCM symptom grading and quantification scores in the two groups are comparable.After treatment,the total score of TCM symptom grading quantification in both groups decreased compared to before treatment(P<0.001),and the total score of TCM symptoms in the abdominal acupuncture group was lower than that in the control group(P<0.001).The total effective rate of TCM symptom improvement in the abdominal acupuncture group was 90.00%,while the total effective rate in the control group was 70.00%,with a statistically significant difference(P<0.05).In the single TCM symptom score,after treatment,the abdominal acupuncture group was significantly lower than the control group in the four TCM symptom scores of fatigue,mental depression or restlessness,dry mouth,and cough(P<0.05).5.Both groups can improve sleep quality and quality of life,but there is no significant difference between the two groups:Before treatment,the PSQI scores and KCCQ-12 scores of the two groups are comparable.After treatment,the KCCQ-12 scores of both groups increased compared to before treatment,while the PSQI scores decreased compared to before treatment.There were statistically significant differences within the group(P<0.001),but there was no statistically significant difference between the groups(P>0.05).6.Safety evaluation:There were no significant deterioration in blood routine and biochemical indicators after intervention in both groups of patients.During the study and trial,there were no adverse events such as needle stagnation,bent needles,and broken needles.There were also no adverse reactions such as needle sickness,subcutaneous hematoma,allergies,and infections in patients,suggesting that abdominal acupuncture treatment is safe.ConclusionAbdominal acupuncture combined with Western medicine anti heart failure treatment can alleviate the anxiety,depression,and traditional Chinese medicine symptoms of patients with anxiety and depression in CHF,and the overall clinical efficacy rate of anxiety,depression,and traditional Chinese medicine symptoms is higher than that of Western medicine anti heart failure treatment alone.Abdominal acupuncture therapy is safe,effective,and has no adverse reactions,and can be applied to patients with chronic heart failure and anxiety and depression.
