Objective To investigate the characteristics of maternal endowment in chronic hepatitis B(CHB)associated Hepatocellular carcinoma(HCC),and to explore the pathogenesis of CHB associated HCC.Methods A total of 172,064 patients with CHB were selected as the case group,the permanent population of Beijing in 2018 was selected as the control group.The patients were divided into different groups according to the time of birth,and the Gestational Five-Yun and Six-Qi endowments were marked.The incidence of CHB in different age groups was compared.Further comparative analysis was made on the characteristics of Five-Yun and Six-Qi endowment in high-risk groups,the internal causes of congenital constitution were determined,and the correlation between Five-Yun and Six-Qi endowment and the occurrence of CHB and post-CHB HCC during pregnancy was explored.Results In terms of CHB:①Among the 24 solar term endowment,both male and female risk factors were autumn,and protective factors were spring,indicating that liver depression and dampness-heat(which can also be combined with spleen dampness)and kidney deficiency and cold were risk factors for CHB,while wood flourishing and qi adaptation and liver qi relaxation were protective factors.②In the endowment of guest yun,the high risk factors of male and female in Yihaishang group and Yihaixia group are Taijin Shaoshui,Shaoshui Taimu and Taimu Shaohuo,and the protective factors are Taitu and Shaojin.The high risk factors of male and female in Bingzi group are Shaojin Taishui and Taitu Shaojin,and the protective factors are Shaohuo,indicating that dampness and heat stagnation in the liver is the high risk factor of CHB disease,and the endowment that is not easy to form stagnation is the protective factor.③In the endowment of guest qi,in Yihaishang group and Yihaixia group,both male and female high risk factors were Taiin Zaiquan Jueyin Sitian Shaoyang Zaiquan and protective factors were Shaoyang Zaiquan Shaoyin Sitian.In Bingzi group,both male and female high risk factors were Shaoyang Zaiquan Shaoyin Sitian Yangming Zaiquan and Yangming Zaiquan Taiyin Sitian Taiyang Zaiquan,and protective factors were Jueyin Sitian Shaoyang Zaiquan,indicating that liver qi suppression and dampness and heat stagnation were the high risk factors of CHB,and the high and uninhibited wood fire were the protective factors.④Among the high-risk and protective groups,there were 41 male high-risk groups,with the highest prevalence in autumn 1976(30.82 ‰),and 37 protected groups,with the lowest prevalence in winter 1998(0.86 ‰).There were 42 female high-risk groups,among which the highest prevalence was 20.83‰ in autumn 1987,and 35 protected groups,among which the lowest prevalence was 0.30 ‰ in autumn 1999.The results showed that the pathogenesis of CHB patients was characterized by liver depression and dampness-heat combined with kidney deficiency cold.In terms of HCC after CHB:①Among the 24 solar terms,the high risk factors for HCC after CHB are spring and summer for men and autumn and winter for women,indicating that the deficiency of healthy qi combined with fire depression is the high risk factor for HCC after CHB,in which fire depression is the main reason and deficiency of healthy qi is the secondary reason.②In the endowment of guest yun,the high risk factors were Shaomu and Shaotu for males,Taishui and Shaoshui for females,and the protective factor is Taitu for both males and females in Yihaishang group,and Taimu for both males and females in Yihaixia group and Bingzi group,which indicates that the high risk factors for HCC are caused by the excess of evil wood and the inhibition of CHB,while the protection factors are caused by the absence of inhibition of pure wood.③In the endowment of guest qi,In Yihaishang group,the male risk factor is Jueyin Zaiquan Yangming Sitian and the protective factor is Taiyang Zaiquan Shaoyang Sitian Jueyin Zaiquan;the female high-risk factor is Taiyin Sitian Taiyang Zaiquan Shaoyang Sitian and the protective factor is Taiyin Zaiquan Jueyin Sitian;the male high-risk factor is Jueyin Sitian Shaoyang Zaiquan Shaoyin Sitian and the protective factor is Jueyin Sitian Shaoyang Zaiquan;the female high-risk factor is Shaoyang Zaiquan Shaoyin Sitian Yangming Zaiquan and the protective factor is Taiyin Zaiquan Jueyin Sitian in Yihaixia group and Bingzi group,indicating that the strong wood fire is suppressed in the liver and then the HCC high risk factor is formed.Liver wood regulation and sufficient spleen and kidney were protective factors.④Among the high-risk and protective groups,there were 20 male high-risk groups,among which the highest prevalence was 2.54 ‰ in spring 1961,and 7 protected groups,among which the lowest prevalence was 0.35 ‰ in winter 1978.There were 42 female high-risk groups,among which the highest prevalence was in spring 1941(1.16 ‰),and 35 protected groups,among which the lowest prevalence was in summer 1968(0.03 ‰).The results showed that the pathogenesis of HCC patients with CHB was characterized by liver depression and dampness-heat combined with kidney deficiency and cold.Conclusions:The congenital constitution of CHB is due to the liver depression and dampness heat caused by the golden soil and mulch,and the kidney deficiency cold.On the basis of liver depression and dampness and heat and kidney deficiency and cold in the original CHB,the higher the wood fire endowment,the higher the wood fire in the body,and the easier it is to transform into HCC.
