Purpose:To observe and evaluate the clinical effect of Zhiyan elimination combined with hypochlorous acid on wound healing after the operation of fire toxic accumulation type perianal abscess,in order to prove that Zhiyan elimination can be applied to the postoperative wound healing of perianal abscess, and has the effectiveness and advantages of promoting healing, and to provide objective basis for its application in wound healing after the operation of anal carbunge.Material and method:A total of 60 patients diagnosed with perianal abscess were enrolled between January 2022 and January 2023.Patients meeting the inclusion criteria were randomly divided into treatment group and control group by SPSS26.0 software, with 30 cases in each group. The postoperative treatment group was washed with hemorrhoid fumigation, once in the morning and once in the evening, and the wound was disinfected with hypochlorite cotton ball until the wound healed completely. The control group was fumigated with potassium permanganate, once in the morning and once in the evening, and the wound was disinfected with hypochlorite cotton ball until the wound healed completely.Outcomes:Postoperative pain score(3,7,14 days),postoperative wound granulation edema score(7,14 days),wound area size(3,7,14 days),measurement of postoperative wound exudate(3,7,14 days),and postoperative wound healing rate comparison(3,7,14 days)were compared between the two groups.Laboratory indicators:1.General bacterial culture: perianal secretions were taken for examination on the 3rd,7th and 14 th day after surgery to detect the positive rate of wound bacteria.2.Inflammatory factors: serum levels of IL-4,IL-6 and IL-10 were detected on the3 rd and 14 th day after surgery.3.Vascular endothelial growth factor: Pathological examination was taken on the 3rd,7th and 14 th day after surgery to detect the content of VEGF in serum.Results:1.General information: There were no statistical differences in age, gender and wound area between the two groups, which showed comparability.2.Wound pain: There were significant differences in postoperative pain at day 3,7 and 14 between the two groups.3.Wound granulation growth: There was no statistical difference on the 7th day after surgery. The wound granulation growth score on the 14 th day after surgery was statistically different.4.Wound area: There were statistical differences between the two groups on the 3rd,14 th day after surgery.5.Wound exudate: The comparison of wound exudate on the 3rd,7th and 14 th day after surgery showed significant differences.6.Wound healing time: 18.80±3.00 in the treatment group,24.33±3.20 in the control group Significant difference.7.Comprehensive efficacy: There was no statistical difference between the two groups.8.General bacterial culture: The positive rate of bacteria in wound exudates showed a decreasing trend9.Inflammatory factors: Interleukin(IL-4),an anti-inflammatory factor, increased first and then decreased.Interleukin(IL-6)was a decreasing trend in the levels of proinflammatory cytokines in both groups.Interleukin(IL-10)was the increasing trend of IL-10 levels in both groups.10.Vascular endothelial growth factor: The levels of vascular endothelial growth factor(VEGF)showed an increasing trend at the 3rd,7th and 14 th day after surgery. Conclusion : Hemorrhoids elimination bath combined with hypochlorite cotton ball disinfection can effectively relieve the wound pain after perianal abscess surgery, play a role in promoting the growth of wound granulation tissue, reduce the positive rate of bacteria on the wound and accelerate healing. In the course of this experimental treatment,no adverse reactions or unsafe events occurred in the patients, which can be clinically promoted in the proctology department.
