Purpose:To retrospectively analyze the clinical data of 68 patients diagnosed with stage I and II idiopathic membranous nephropathy in the inpatient department and outpatient department of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine,and observe the efficacy and safety of Tao chen mo shen jian based on the theory of "Qi,blood and water" in the treatment of patients with spleen-kidney deficiency and phlegm-stasis interjunction IMN.To elucidate its characteristic,advantage and mechanism of action,and to provide new ideas for clinical diagnosis and treatment of membranous nephropathy.Methods : By retrospective cohort study,68 patients with stage I and II idiopathic membranous nephropathy with spleen-kidney deficiency and phlegm-stasis interconnection were enrolled from the outpatient or inpatient department of the First Affiliated Hospital of Liaoning University of Traditional Chinese Medicine.All patients had complete clinical,pathological and laboratory data.33 cases were divided into treatment group and 35 cases in the control group.The control group was treated with basic therapy(including daily living and diet control)plus ARB(including Losartan potassium tablets 50 mg orally once daily,Valsartan capsules 80 mg orally once daily,or Telmisartan tablets 40 mg orally once daily,etc).The treatment group was given the control group treatment + tao chen mo shen jian for 12 weeks.The 24-UTP,ALB,blood lipid,liver and kidney function,and TCM syndrome symptoms and curative effects of all patients were observed before treatment,6 weeks and 12 weeks of treatment.Compared with the control group,the efficacy and safety of "Tao chen mo shen jian" in the treatment of spleen-kidney deficiency,phlegm-stasis interconnection type I and II IMN were analyzed.Results:A total of 68 patients were collected in this study,including 35 patients in the control group and 33 patients in the treatment group.There was no significant difference in the general data between the two groups(P > 0.05),which was comparable.1.Comparison of 24-UTP,ALB,CHOL and TG: compared with the pre-treatment period,24-UTP,CHOL and TG decreased significantly(P < 0.05)at 6 and 12 weeks of treatment,and ALB increased significantly(P < 0.05)at 6 and 12 weeks of treatment in the two groups;at 6 and 12 weeks of treatment,the treatment group was stronger than the control group in lowering 24-UTP,raising ALB,lowering TG,and lowering CHOL at 6 and 12 weeks of treatment(P < 0.05).2.Comparison of Scr,BUN,AST and ALT: Compared with the pre-treatment period,there was no statistical difference in the levels of Scr,BUN,AST and ALT between the two groups at 6 and 12 weeks of treatment(P > 0.05).There was no statistical difference in Scr,BUN,AST,and ALT in the treatment group compared with the control group at 6 and 12 weeks of treatment(P > 0.05).3.Comparison of TCM symptom scores: compared with the pre-treatment period,there was a statistically significant difference in the level of TCM symptom scores between the two groups at 6 and 12 weeks of treatment(P < 0.05);the TCM symptom scores of the treatment group were significantly better than those of the control group at 6 and 12 weeks of treatment(P < 0.05).4.Changes in TCM efficacy: the total effective rate of the treatment group was 87.8%,and the total effective rate of the control group was 48.6%,with statistically significant differences between the treatment group and the control group(P < 0.05).5.Comprehensive efficacy change comparison: the total effective rate of the treatment group was 78.8%,the total effective rate of the control group was 42.8%,the effect of the treatment group than the control group was obvious,with statistical differences(P < 0.05).Conclusion: Tao chen mo shen jian has good clinical efficacy in treating idiopathic membrane nephropathy with spleen-kidney deficiency and phlegm-stasis interconnection.This retrospective analysis observed previous clinical data and analyzed that it has significant effects in controlling urine protein,increasing albumin and delaying the progression of renal function,and is safe without significant side effects.
