Objective: 1.To study the constitution types of CHD patients with blood-stasis syndrome,classify the constitution of CHD with blood-stasis syndrome,and explore the correlation between CHD with blood-stasis syndrome and constitution.2.The candidate genes were detected by fluorescence quantitative PCR to verify the relationship between the candidate genes and CHD with bloodstasis syndrome and its susceptible constitution.Methods: 1.171 inpatients with CHD with blood-stasis syndrome and 23 healthy people who underwent physical examination in the Department of Cardiology,the Second Affiliated Hospital of Hunan University of Traditional Chinese Medicine from January 2022 to December 2022 were collected.The constitution of patients with CHD with blood-stasis syndrome included in the study was classified.The patients’ age,gender,body mass index,blood lipid level,high-sensitivity Creactive protein,blood type homocysteine,and traditional Chinese medicine constitution type were analyzed,and the clinical conclusions were drawn.2.The susceptible constitution was divided into groups according to the constitution type,and the healthy people were set as the control group.The levels of ABCA1 and ACSL4 in peripheral blood of each group were detected by RT-PCR and Elisa,and the statistical analysis was performed by SPSS 25.0 software to draw conclusions.Results:1.TCM constitution distribution research results(1)A total of 171 patients with CHD with blood-stasis syndrome were investigated in this study.Among them,113 cases had a single constitution,and the top three constitutions in the single constitution were phlegm-dampness constitution,blood-stasis constitution,and qi deficiency constitution.There were 58 cases with both constitutions,including 39 cases with two constitutions and 19 cases with three or more constitutions.(2)There was no significant difference in age and course of disease between the groups with different constitution types(P > 0.05).There was no statistically significant difference between different types of constitution and gender(P > 0.05).(3)The levels of HDL-C,LDL-C and CHOL in the three susceptible groups were significantly lower than those in the control group,and the levels of LDL-C and CHOL in the three susceptible groups were significantly higher than those in the control group(P < 0.05).The levels of HDL-C,LDL-C and CHOL in the three susceptible constitution groups were compared:(1)The levels of HDL-C in the phlegm dampness constitution group and the blood stasis constitution group were lower than those in the qi deficiency constitution group,and the difference was statistically significant(P < 0.05),but there was no significant difference between the phlegm dampness constitution group and the blood stasis constitution group(P > 0.05);(2)The CHOL of phlegmdampness and blood-stasis constitution group was higher than that of qi deficiency constitution group,and the difference was statistically significant(P < 0.05),but there was no statistically significant difference between phlegm-dampness constitution group and blood-stasis constitution group(P > 0.05);(3)The differences of LDL-C among the three groups were statistically significant(P < 0.05),and the phlegm-dampness type group > blood stasis type group > qi deficiency type group.(4)Compared with the healthy group,the levels of BMI,hs-CRP and Hcy in the three susceptible groups were significantly higher than those in the normal group(P < 0.05).There was no significant difference in BMI between the Qi-deficiency and blood-stasis constitution groups and the control group(P > 0.05),while the phlegm dampness constitution group had a significantly higher BMI than the control group(P < 0.05).The BMI,hs-CRP and Hcy of the three susceptible constitution groups were compared:(1)The BMI of the phlegm dampness constitution group was higher than that of the qi deficiency constitution group and the blood stasis constitution group,and the difference was statistically significant(P < 0.05),but the BMI of the qi deficiency constitution group and the blood stasis constitution group was not statistically significant(P > 0.05);(2)The levels of hs-CRP in blood stasis constitution group and phlegm dampness constitution group were higher than that in qi deficiency constitution group,the difference was statistically significant(P< 0.05),the levels of hs-CRP in blood stasis constitution group and phlegm dampness constitution group were not statistically significant(P > 0.05);(3)The level of Hcy in the phlegm-dampness group was higher than that in the qi deficiency group and the blood stasis group,and the difference was statistically significant(P < 0.05).The level of Hcy in the qi deficiency group and the blood stasis group was not statistically significant(P > 0.05).2.The results of ABCA1 and ACSL4 genes were detected by RT-PCR and Elisa(1)RT-PCR and Elisa were used to detect the levels of ABCA1 in the peripheral blood of the CHD with blood-stasis syndrome susceptible group and the healthy control group.The levels of ABCA1 in the three CHD with blood-stasis syndrome susceptible groups were higher than those in the control group,and the difference was statistically significant(P < 0.05).The level of ABCA1 in the phlegm-dampness constitution group was significantly higher than that in the Qi-deficiency constitution group and the blood-stasis constitution group(P < 0.05),but there was no significant difference between the Qi-deficiency constitution group and the blood-stasis constitution group(P > 0.05).(2)RT-PCR and Elisa were used to detect the levels of peripheral blood ACSL4 in the CHD with blood-stasis syndrome susceptible group and the healthy control group,respectively.The levels of ACSL4 in the three susceptible groups were higher than those in the control group,and the difference was statistically significant(P < 0.05).The levels of ACSL4 in the phlegm-dampness constitution group and the blood-stasis constitution group were significantly higher than that in the Qi-deficiency constitution group(P < 0.05),but there was no significant difference between the phlegm-dampness constitution group and the blood-stasis constitution group(P > 0.05).Conclusions:1.The three most common constitutions of CHD patients with blood-stasis syndrome were qi deficiency constitution,blood-stasis constitution and phlegm dampness constitution.2.Blood lipid level,hs-CRP and Hcy are related to CHD with blood-stasis syndrome,and hs-CRP and Hcy are independent risk factors for CHD.3.In CHD patients with blood-stasis syndrome,blood lipid level and hs-CRP are related to phlegm-dampness and blood-stasis constitution,and blood type homocysteine is related to bloodstasis constitution,and a strong correlation between BMI and phlegm-dampness.4.Both ABCA1 and ACSL4 are susceptible genes for CHD with blood-stasis syndrome.The expression of ABCA1 in phlegm-dampness constitution is higher than that in blood stasis constitution and qi deficiency constitution,and the expression of ACSL4 in phlegm-dampness constitution and blood stasis constitution is higher than that in qi deficiency constitution.
