| Objectives:This study aims to develop a questionnaire of influencing factors of medication adherence in hypertension patients based on health promotion model, investigate the status of medication adherence in elderly hypertension patients in the community, and analyze the influencing factors of medication adherence in elderly hypertension patients under the theoretical guidance of health promotion model. It lays a scientific foundation for the related application of health promotion model and the future intervention research of disease management in elderly hypertension patients. Methods:With health promotion model as the theoretical framework, the first draft of the questionnaire is formed through literature review,semi-structured interview and expert letter inquiry. It is convenient to select the community elderly hypertensive patients who meet the inclusion and exclusion criteria for pre-investigation,conduct a preliminary test on the first draft of the questionnaire, and form the pre-trial version of the questionnaire after adjustment. After formal testing, the project analysis will be carried out to check the relevance and reliability of the individual items of the questionnaire;content validity and structural validity were used for validity testing;the internal consistency test was used to test the reliability test, and the questionnaire items were adjusted according to the test results to form the final version of the questionnaire on the influencing factors of medication adherence of hypertensive patients based on the health promotion model.Using convenient sampling to select 360 community elderly hypertension patients meet the inclusion and exclusion criteria for status quo investigation, the results using mean ± standard deviation, frequency, ratio to describe the general data of community elderly patients with hypertension and medication adherence, using t test, variance analysis, correlation analysis and multiple linear regression gradually analyze the influencing factors of community elderly hypertension patients medication adherence.Results:1.In this study, questionnaire on factors influencing medication adherence in hypertensive patients based on health promotion model consisted of 7 dimensions,42 items, namely perceived benefits of action(7 items),perceived barriers to action(11 items),perceived self-efficacy(5 items),activity-related affect(4 items),interpersonal influences(4 items),situational influences(3 items),commitent to a plan of action(8 items). The overall Cronbach’s α of the questionnaire was 0.927 and Cronbach’s α of each dimension was0.747~0.918.The item-level content validity index(I-CVI)of the questionnaire was 0.875~1,and scale-level content validity(S-CVI)of the questionnaire was 0.804.The exploratory factor analysis results extracted 7 factors, and the cumulative variance contribution rate was61.009%,and the factor load of each item in its common factors was> 0.4.In conclusion, questionnaire on factors influencing medication adherence in hypertensive patients based on health promotion model developed in this study has good reliability and validity.2.In this study, the current situation of medication adherence of the community elderly hypertension patients showed that the medication adherence score was(5.28 ± 1.81).3.The results of multiple linear regression of factors influencing medication adherence in community elderly hypertension in this study showed that perceived barriers to action, frequency of regular blood pressure measurement, situational influences, age, perceived self-efficacy,occupation type, monthly income, educational level and course of disease entered the regression equation(P <0.05).Conclusions:1.In this study,we designed a questionnaire on influencing factors of medication adherence in hypertensive patients based on health promotion model. This questionnaire has good reliability and validity and can be used to investigate the influencing factors of medication adherence in elderly hypertensive patients in the community.2.The overall score of medication adherence among community elderly hypertensive patients was at a low level and their medication adherence was poor.3.The results in this study showed that perceived barriers to action, frequency of regular blood pressure measurements, situational influences, age, perceived self-efficacy,occupation type, monthly income, educational level, and disease course were influencing factors for medication adherence in elderly hypertensive patients in the community. |
