Purpose:By collecting the information of four diagnoses of elderly frailty patients with sleep disorders aged 60 years and over,and analyzing the traditional Chinese medicine(TCM)syndrome information using systematic clustering method,we hope to provide a basis for clinical diagnosis and treatment of elderly frailty patients with sleep disorders.Material and method:A cross-sectional study was conducted.The elderly patients were collected from the Geriatrics department,Cardiology department,Endocrinology Department and other departments of Liaoning Provincial Hospital of Traditional Chinese Medicine,and the rehabilitation department and cardiac rehabilitation department of Liaoning Rehabilitation Center.According to the TCM literature and clinical common symptoms,the information collection table of the four diagnosis of TCM was designed to collect the information of the four diagnosis of TCM in elderly inpatients who met the diagnosis of elderly frailty and sleep disorders.The collected information of four diagnoses was input into the WPS software,and the value was assigned and the database was established.Finally,the data were imported into SPSS 26.0 software,and the frequency and percentage of each symptom were statistically analyzed.The distribution characteristics of TCM syndromes were analyzed by cluster analysis method,and finally the TCM syndrome types of elderly patients with frailty combined with sleep disorders were determined.Results:1.A total of 447 elderly inpatients were evaluated in this survey,including 340 elderly patients with frailty,accounting for 76.06% of the total number of patients,68 patients with pre-frailty,accounting for 15.21% of the total number of patients,and 39 patients with non-frailty,accounting for 8.37% of the total number of patients.2.A total of 340 elderly patients with frailty were screened for sleep assessment,of which 212 cases had sleep disorders,accounting for 62.4% of the total number;There were 128 elderly frailty patients without sleep disorders.They accounted for 37.6% of the total.3.Among the 212 elderly patients with frailty and sleep disorders,80 were male,and the proportion of male patients with frailty and sleep disorders was 37.7%.There were 132 female patients,and the proportion of female patients with frailty combined with sleep disorders was 62.3%.The incidence of sleep disorders in female elderly patients with frailty is higher than that in male patients.Among the 212 elderly patients with frailty combined with sleep disorders,the youngest patient was 60 years old,the oldest was 99 years old,the median age was 72,and the average age was73.46.Among the patients aged 60-69 years,64(37.7%)had frailty combined with sleep disorders.Among the patients aged 70-79 years old,there were 80 patients with frailty and sleep disorders,accounting for 62.3%.Among the patients aged 80-89 years,49(23.1%)had frailty combined with sleep disorders.There were 19 elderly frailty patients over 90 years old with sleep disorders,accounting for 9%.It can be seen that the age distribution of elderly frailty patients with sleep disorders is the highest in the 70-79 age group.4.Among 212 elderly patients with frailty and sleep disorders,there were 4 patients with BMI of emaciation,accounting for 2%;There were 92 patients with normal BMI,accounting for 43%;There were 67 patients with overweight,accounting for 32%;There were 49 patients whose BMI was obese,accounting for 23%.5.Among the 212 elderly inpatients with frailty and sleep disorders,the top 15 medical information items were palpitation,amnesia,red tongue,hot flashes,tinnitus,waist and knee soreness,deafness,dry mouth,thin pulse,night sweat,low back pain,chest tightness,poor appetite,loose stool and fatigue.6.After cluster analysis,the above four cluster results were summarized as Yang deficiency and blood stasis syndrome,liver depression and blood deficiency syndrome,heart and spleen deficiency syndrome,kidney Yin deficiency and fire hyperactivity syndrome.7.Among the elderly patients with frailty combined with sleep disorders,the female patients were significantly more than the male patients,about 1.5 times as much as the male patients.Moreover,there is a significant correlation between elderly frailty combined with sleep disorders and gender,and there is a positive correlation between them.This indicates that women are more likely to suffer from sleep disorders than men in elderly patients with frailty.The incidence of elderly frailty combined with sleep disorders is significantly correlated with age.The older the elderly frailty patients are,the more likely they are to suffer from sleep disorders.There is no correlation between the prevalence of frailty combined with sleep disorders and BMI in elderly inpatients.Conclusion:1.The elderly frailty combined with sleep disorders can be divided into four TCM syndromes: Yang deficiency and blood stasis syndrome,liver depression and blood deficiency syndrome,heart and spleen deficiency syndrome,kidney Yin deficiency and fire hyperactivity syndrome.2.In this study,female frail elderly patients were more likely to suffer from sleep disorders than male frail elderly patients;With the increase of age,elderly patients with frailty are more likely to suffer from sleep disorders.There is no correlation between the incidence of elderly frailty combined with sleep disorders and BMI.
