At present,hypertension presents a severe situation of "one high and three low" with high prevalence,low awareness,low treatment rate and low control rate.Aiming at how to achieve the integration of medical and prevention for hypertension patients,effectively reduce the incidence rate of hypertension and improve the control rate of hypertension patients,Guiyang City has explored and established a medical and prevention integration system that integrates city county township village four levels of hypertension diagnosis and treatment with chronic disease management.This study explores the construction path,operation status,and effectiveness of the hypertension grading diagnosis and treatment system in Guiyang City,identifies the difficulties and shortcomings that have arisen during the implementation process of the system,summarizes and analyzes the reasons,and proposes path optimization strategies.Based on the existing foundation,it discusses the effective and suitable integrated management model of hypertension diagnosis and treatment and prevention(medical prevention integration)for promotion,To provide reference for establishing a medical prevention integrated management system for hypertension in more regions.Through literature analysis,data organization,and departmental consultation,it was learned that Guiyang City has established a four level basic diagnosis and treatment system at the city county township village level with the goal of building a unified management system for hypertension patients.It has established an operational system that integrates personnel training,two-way referral,unified management and information sharing,and unified drug allocation and reimbursement,in order to achieve the management goals of the entire range,entire course,and entire cycle of hypertension patients in the city.Through on-site investigation,interviews,questionnaires,and data collection,it was found that the management rate,satisfaction rate of blood pressure control,and medication taking rate of hypertension patients in Guiyang City are increasing year by year.However,there are also problems in the system construction,such as incomplete organizational design,slow system construction process,unclear operational mechanism effectiveness,low patient awareness rate,and low management cooperation.Analysis shows that incomplete supporting policies at the government level,difficult to break profit driven motives at the hospital level,lack of incentive mechanisms at the doctor level,and unwillingness to implement initial consultations at the patient level have resulted in insignificant results in the operation of hypertension grading diagnosis and treatment.Through positive and negative arguments,a more optimized regional integrated management system and path for hypertension prevention and treatment are derived from four levels: government,hospital,doctor,and patient.The government should optimize and improve the design of institutional systems,focus on improving the service capabilities of grassroots medical institutions,build a medical information sharing platform,establish a system assessment system,and improve relevant regulatory mechanisms,Strengthen the promotion and guidance of graded diagnosis and treatment related content throughout society;At the hospital level,the government should increase financial input,improve the awareness of prevention and health care,promote the return of the public welfare nature of the hospital,expand the core business of the hospital,and provide more high-quality medical services;At the doctor level,we should improve the talent incentive system,increase the enthusiasm of medical staff,and also encourage them to enhance their core medical service capabilities,actively promoting the sinking of new businesses and technologies;At the patient level,it is necessary to strengthen publicity and guidance,promote the establishment of correct health management concepts,and establish incentive mechanisms to induce patients to seek medical treatment rationally.
