Background:Enuresis is one of the most common clinical diseases of children. The patient children would be badly affected in the development of mentality and personality and experience a life of poor quality. Accordingly, an effective intervention in the early enuresis is of vital significance to the society and a tough task at the same time. Recently, many researches from different perspectives have been done. Although the pathogenesis is not thoroughly obtained, the detrusor instability (DI) has been the main focus domestically. DI is a common vesicourethral dysfunction. The increase of ICCs cells is a possible reason for causing DI. As a NANC neurotransmitter, Nitric oxide (NO) also participates in the regulation of bladder function.In the meantime, variation of nerve growth factor levels can affect the distribution of pathologicalbladder nerves.Traditional Chinese medicine has a long research history on this disease and abundant experience in therapy. Research in traditional Chinese medicine can be a beneficialsupplement in conquering this disease. Former clinical observation tells a good effect of the method of relieving lung for dieresis. This study employs Yuebi decoction as the interveneprescription, exploring the mechanism of how the Yuebi decoction on the enuresis.Study Purpose:This dissertation discusses how the Yuebi decoction affects the unstable bladder model of enuresis. A rigorousdemonstration of the correctness of theories in traditional Chinese medicine such as"Method of Relieving lung for dieresis","Method of Treating Diseases in Lower Part by Managing Upper" and "the lung is the upper source of water circulation"Study Procedure:A number of108Wistar rats are divided into two groups, bladderoutletobstruction-operated group and sham-operated group. After six weeks, divided the DI rats into Yuebi decoction with original dosage, Yuebi decoction with high dosage, Yuebi decoction with middle dosage, Yuebi decoction with low dosage,Suoquan Wan, model group, and the sham-operated group. So there was seven groups, and10-13rats in every group. The effects of Yuebi decoction on DI model were observed. After14days of feeding the rats, we observed the bladder wet weight, bladder abscess, bladder calculus for evaluation the effect of drugs; bladder ICCs by Immunohistochemistry and Western Blot method, NOS by ELISA and Immunohistochemistry method, and NGF by ELISA and Immunohistochemistry. By above point of view, researching Yuebi decoction hampering DI progress possible mechanism.Results:1.Six weeks after the operation, filling bladder pressure detection was conducted by the urinary dynamics detector. In the sham-operated group, the occurrencerate of DI is16.7%. In the operation group, the occurrencerate of DI is77.8%.15of81in this group have urinarytractinfection or die because of urine leakage.2.1n the model control group, one has abscess bladder and two have bladder calculus, In the low-dose Yuebi decoction group, one has abscess bladder and another one has bladder stone. There is no abscess bladder or bladder stone record in other groups. Compared with sham-operated group, the bladder wet weight of model control group is significantly high, demonstrating a obvious statisticalsignificance (P<0.01). Compared with the model control group, the bladder wet weight of Yuebi decoction group is significantly low, demonstrating a obvious statisticalsignificance (P<0.05)3.1nfluence on the ICCs cells: Compared with sham-operated group, the ICCs of model control group is significantly high, demonstrating a obvious statisticalsignificance (P<0.05) Compared with the model control group, the ICCs of Yuebi decoction group is significantly low, demonstrating a obvious statisticalsignificance (P<0.05),and with which showing a declining trend of ICCs.4. Influence on the NOS:(1)By Immunohistochemistry method from bladder tissues:Compared with sham-operated group, the NOS of model control group is significantly high, demonstrating a obvious statisticalsignificance (P<0.05). Compared with the model control group, the NOS of Yuebi decoction group is significantly low, demonstrating a obvious statisticalsignificance (P<0.05),and with which showing a declining trend of NOS.(2)By ELISA from serum:Compared with sham-operated group, the NOS of model control group is significantly high, demonstrating a obvious statisticalsignificance (P<0.05). Compared with the model control group, the NOS of Yuebi decoction group is showing a declining trend of NOS.5. Influence on the NGF:(1)By Immunohistochemistry method from bladder tissues:Compared with sham-operated group, the NGF from hypothalamus tissues of model control group is significantly low, demonstrating a obvious statisticalsignificance (P<0.01). Compared with the model control group, the NGF from hypothalamus tissues of Yuebi decoction is significantly high, demonstrating a obvious statisticalsignificance (P<0.05)(2)By Immunohistochemistry method from hypothalamus tissues:Compared with sham-operated group, the NGF from hypothalamus tissues of model control group is significantly low, demonstrating a obvious statisticalsignificance (P<0.01) Compared with the model control group, the NGF from hypothalamus tissues of Yuebi decoction is significantly high, demonstrating a obvious statisticalsignificance (P<0.05)(3) By ELISA from serum:Compared with sham-operated group, the NGF from serum of model control group is significantly low, demonstrating a obvious statisticalsignificance (P<0.01). Compared with the model control group, the NGF from serum of Yuebi decoction is significantly high, demonstrating a obvious statisticalsignificance (P<0.05)Conclusions:1.Yuebi decoction can dramatically improve the bladder dysfunction caused by detrusor instability (DI)2.1n a detrusor instabilitybladder, the number of ICCs cells and NOS increased and the NGF in serum, hypothalamus and bladder tissues decreased in its content.3.Yuebi decoction can lower the number of ICCs cells and NOS, and escalate the NGF levels, which is the key to explain the mechanism of therapy.
