It has many advantages on treatment for diseases, such as convenient use, spreading easily, becoming effective rapidly and reliable effect, no bad response and ill effect, and so on. The purpose of the research is to exploring the mechanism of proctoclysis of Tongfuxingshen liquid treating unconscious patients with cerebral hemorrhage and encephaledema, cerebral vessels permeability and expression of HSP70, HO-lmRNA after cerebral hemorrhage in rats by TCM theory and clinical, experiment. Providing the theoretic and clinical, experimental evidence for it, and exploring the new administration ways to treating acute intracerebral hemorrhage. Part One : theory researchBy reviewing the survey of external therapy on stroke in successive dynasties and clinical application of the proctoclysis of Chinese traditional medicine in resent ten years, we analyse and summarize the headstream and actuality of the Tongfu therapy on stroke coma. The dissertation emphasize the therapy on critical acute ICH is still a difficult problem, and it is urgent to apply the other administration ways on serious symptom of acute stroke except traditional oral administration. The proctoclysis of Chinese traditional medicine is a new administration way and has good effect on acute intracerebral hemorrhage in resent ten years, which is worthy of being research. Tongfuxingshen capsule, created according to professor Liu Maocai clinical experience, is an effective drug on acute ICH and one of drugs in the Nations tackle key problems inscience and technology. In order to improve the effect of treatment of acute ICH, The proctoclysis of Tongfuxignshen liquid is applied.The path-physiological mechanism of ICH is complicated, which includes the high intracranial pressure caused by hematom mechanical pressure, the lesion by the decrease of regional cerebral blood flow surrouding the hematom, even regions adjacent to the hematom, Meanwhile, the materials released from hematom including ferrohemoglobin, thrombase, plasma protein and platelet, leucocyte, glusate not only directly injury neurons but also blood brain barrier. On the other hand, the body protection mechanism against lesions after ICH is found in resent years; Heat shock response is one of it, such as HO-1 and HSP70. For this reason, it is a new pathway to explore some methods, drugs and administration ways to improve the expression HSP70 and HO-1 for preventing the injury after ICH. Part Two : clinical researchIn this research, sixty-one patients in unconscious state after hypertensive intracerebral basal ganglia hemorrhage was randomly divided into two groups: The proctoclysis group (3leases) was treated with Tongfuxingshen liquid but the control group (SOcases) treated with Tongfuxingshen capsul for seven days. Otherwise, the symptomatic therapy was used simultaneously. The curative effects such as the time of the patients from unconsciousness to consciousness state, the scores of Glasgow-Pittsburgh coma after treated seven days, and the scores of nervous functional defects after treated fourteenth days were observed. Results: By the therapy the time of the patients from unconsciousness to consciousness (4.94 days) in proctoclysis group was significantly shorter than that (6.9 days) in the control group. After seven days and fourteenth days therapy the increase in the scores of Glasgow-Pittsburgh coma and the total effective rates in proctoclysis group were superior to those in control group.
