1. The paper explores the regularity of the treatment based on differential diagnosis which ancient and contemporary medical experts applied to childhood persistent and chronic diarrhea diseases by collating the literature concerned systematically.2. The paper also approaches the regularity of differential diagnosis about childhood persistent and chronic diarrhea diseases from an analysis of the characteristics and influencing factor of the syndromes of the diseases, the gradating regularity of the syndromes and their pathology through the clinical investigation. The study indicates that the primary syndromes of the diseases are the Dysfunction in Transport due to Spleen Deficiency (DTSD) and Deficiency of both Spleen-yang and kidney-yang (DSKY), mainly DTSD. More of DTSDare seen in childhood persistent diarrhea diseases, but the number of DSKY increases gradually with the course of the diseases prolonged. The inversion of the syndromes is influenced by the age, the course of the diseases, the malnutrition, the function of digestion and absorption. The pathology is deficiency with excess concomitantly and the location of the lesion of DTSD is at spleen and stomach. DSKY is located at spleen, stomach and kidney and itspathology is weakness and coldness. Synthesizing the results of cluster analysis, frequency count of pulse condition and symptoms, principal component analysis and discriminatory analysis indicates that the characteristic symptoms of DTSDprimarily are loose stools, food wastes concomitantly, abdominal distension, sallow complexion, spontaneous perspiration, white and greasy fur, and the characteristic symptoms of DSKY primarily are diarrhea with undigested food or nothing mixed up with in the stool, pallor, the eyeballs emerging while sleeping, cold limps, thin and white fur, and that the common characteristic symptoms of both the syndromes are mainly anorexia, emaciation, flaccid skin, lassitude, pale tongue, small and weak pulse.3. The total effective rates and recovery rates of JianPiZhuYun Formula (JPZYF) -jianyun granule on childhood persistent and chronic diarrhea with DTSD, are superior to those of shenlingbaizhu powder applied in the control group (P<0.005,0.01 respectively). Jianyun granule can improve the child patient's digestive and absorptive function and ameliorate his anemia and hypoalbuminemia. The observation of scanning electron microscope (SEM) and transmission electron microscope (TEM) reveals that jianyun granule can significantly repair the ultrastructure of injured intestinal mucosa, and is superior to the shenlingbaizhu powder. In the study of Ag-NOR stain and EGFR expression, we found that the mechanism of the restorative effect of jianyun granule on injured intestinal mucosa lie mainly in improving the activity of repair cells, which have close relation with the increase of cytogenesis factor in local microenvironment.
