Objective To study the anatomic characteristic of left common iliac vein and the caval bifurcation, study the histology of the compressed left common iliac vein and the intravenous band,discuss the anatomic basis of the iliac vein compression syndrome and the reason of the intravenous band formation.Methods 25 samples were colorated with Van-Gieson,Verhouff and HE technique.The distribution and content of smooth muscle,collagenous fiber and elastic fiber were observed and measured under optical microscopy.We also measured the thickness of the front wall and hind wall of compressed left iliac vein,the vein wall of boundary of band and vein,and the thickness of their intima. 25 samples were colorated with Van-Gieson,Verhouff and HE technique.The distribution and content of smooth muscle,collagenous fiber and elastic fiber were observed and measured under optical microscopy.We also measured the thickness of the front wall and hind wall of compressed left iliac vein,the vein wall of boundary of band and vein,and the thickness of their intima.Result 1.The angle of the left iliac vein and vena cava was bigger than the angle of theright iliac vein and vena cava. The left iliac vein was compressed by the righ common iliac artery and the lower lumber vertebrace. the longitudinal diameter of the compressed left iliac vein reduced.The transverse diameter reduced in the compressed left iliac vein with band formation,however it increased without band formation.The occurrence of band was 25%.2.Smooth muscle,collagenous fiber and elastic fiber were found in intravenous band,their arrange were irregular, smooth muscle reduced(P<0.01)and collagenous fiber increased(P<0.05) compared with the right iliac vein,however smooth muscle increased(P<0.05) at the boundary of the band and vein.The intima of the front wall and hind wall of the compressed left iliac vein increased(P<0.05) as well as the intima of the boundary of band and vein (P<0.01) compared with the right iliac vein.The thickness of the compressed left iliac vein front wall reduced(P<0.01), however at boundary the thickness of the vein wall increased(P<0.01).Conclusion 1.The anatomic basis of the iliac vein compression syndrome lies in righ iliac artery and the lower lumber vertebrace compressing the left iliac vein and intravenous band formation,they results in the changes of the transverse and longitudinal diameter in the compressed left iliac vein, and band formation affects the veinal diameter obviously.2.Intravenous band developed during the patient's lifetime and not as congenital disturbances.It due to long time compression and result in excess proliferate and fibrosis at the intima of the compressed left iliac vein.
