Objective: The aim of the study was to measure the level of cytokines in serum of patients with ulcerative colitis and study the relationship between cytokines and lesion of ulcerative colitis and degree of the disease. We also studied the expression of matrix metalloproteinase-3 (MMP-3), matrix metalloproteinase-9 (MMP-9), Bcl-2 and Bax in inflamed mucosa and noninflamed mucosa of ulcerative colitis.Method: The serum of patients with ulcerative colitis was collected from May 2001 to Nov 2001, and stored frozen at -70(C. The level of IL-1(, IL-2, IL-6, IL-8, IL-12p40 and TNF-( were measured with ELISA. The IL-1(, IL-2, IL-6, IL-8, IL-12p40 and TNF-( ELISA kits were purchased from Diaclone, France. Date analysis: Generate a linear curve by plotting the average absorbency on the vertical axis versus the corresponding cytokine standard concentration on the horizontal axis. The amount of cytokine in each sample is determined by extrapolating OD values to cytokin concentrations. The expression of MMP3, MMP9, Bcl-2 and Bax in mucosa of ulcerative colitis were detected by immunohistology.Statistical analysis: All dates were analyzed by Student's t-test, linearityregression analysis and Chi-square test. P values less than 0.05 was regarded as significant difference.Results: Sixty-four cases of serum of patients with ulcerative colitis were collected. The average year of patients was 43.5(11.9 years old, from 18 to 65 years old. Among them male were 24 cases and female were 40 cases. The disease involved whole colon of 13 cases, left colon of 7 cases, rectum and sigmoid colon of 19 cases, rectum only of 13 cases and no lesion of 12 cases。The degree of the lesion were including 0 degree of 12 cases, 1 degree of 14 cases, 2 degree of 19 cases and 3 degree of 19 cases. Colonic mucosa of thirty-one cases of patients with ulcerative colitis was collected.The average level of IL-1( in serum of patients with ulcerative colitis was 10.8(14pg/ml, and that of normal control was 1.8(0.57pg/ml. There was significant difference between two groups (p<0.01). The concentration of IL-1( in serum of patients with different lesion of diseases was that healing stage was 6(8.3pg/ml, rectum lesion only was 9(11pg/ml, rectum and sigmoid lesion was 8.3(7.8pg/ml, left colon disorder was 19.8(14.6pg/ml, whole colon disorder was 15.5(23.3pg/ml. There is a trend that the more the lesion was, the higher the concentration of IL-1( was. The level of IL-1( in serum of patients with left colon disorder was higher than that of patients of healing stage under endoscopic investigation (p<0.05). The concentration of IL-1( in serumof patients with different degree of mucosa damage according to endoscopic examination was that degree 0 was 6(8.3pg/ml, degree 1 was 6.2(6.2pg/ml, degree 2 was 11.2(12.6pg/ml and degree 3 was 16.9(19.7pg/ml. The level of IL-1( was higher in degree 2 and degree 3 colitis than that of in degree 0 and degree 1 damages (p<0.05). The concentration of IL-1( in serum of patients with degree 0 damage was higher than that of normal controls (p<0.02). The concentration of IL-1( in serum of patients of first attack was 16.1(18.2 pg./ml, which was higher than that of recurrent patients of 8(9.2pg/ml (p=0.031). There were 19 patients with ulcerative colitis received routine treatment with 5-ASA for 2 months. Before treatment the concentration of IL-1( was 11.9(11.8pg/ml, and decreased to 7.4(7.8pg/ml after treatment. There was significant difference (p=0.017).The average level of IL-2 in serum of patients with ulcerative colitis was 14.2(3.6pg/ml, and that of normal control was 12.3(2.1pg/ml. There was significant difference between two groups (p<0.01). The concentration of IL-1( in serum of patients with different lesion of diseases was that healing stage was 14.5(1.7pg/ml, rectum lesion only was 12.5(1.5pg/ml, rectum and sigmoid lesion was 13.8(1.8pg/ml, left colon disorder was 13.1(5.2pg/ml, whole colon disorder was 16.7(5.6pg/ml.
