IntroductionPrimary hyperparathyroidism ( PHPT) is one of the most dangerous disease to mankind and both the morbidity and mortality of which increasingly rose in the world. In the developed countries, its morbidity was about 1.3 percent, and up to 100,000 new cases of PHPT are diagnosed each year. But there are 16,000 cases should be operated in American every year. Some authors reported more than 100 cases of PHPT, for example, Gaz et al. described 1200 cases which were operated in 1987 ,and Hodback et al. demonstrated 896 cases in Sweden in 1995. PHPT was one of the most dangerous disease, which is on the third list of the endocrinology disease. In recent year, PHPT has been reported 760 cases in China, so PHPT is a disease with significant clinical importance.Although parathyroidecomy remains the mainstay of treatment for PHPT, some authors are questioning whether operative treatment of PHPT is necessary in " mild" case of the disease without any evidence of its complication and whether parathroidecomy, early on, is justified to prevent long-term complications associated with PHPT, especially in the current economic environment . The bone disease is on the top list of PHPT. Such question have stimulated research in the basic mechanisms and studied the characteristics of osteopathy in PHPT on CT, MRI and histopathologic.The most important x-ray image is the resorption of the cortical bone and osteitis fibrosa cystica . when the bone calcium lost at 30 ~50 percent, the bone resorption was found by x-ray examination . But MRI and CT is superior to con-vertional radiography in imaging diagnosis, and it might be a reliable methods for the study of early osteopathy in PHPT .At present, it was not reported about apoptosis and proliferation of parathy-raid cell in PHPT and the correlation between its expression and CT, MRI and histopathologic in osteopathy . There were no reports about the relationship between the characteristics of osteopathy on CT, MRI and histopathologic in PHPT. The animal model of PHPT was eatabilished by high phosphate diet induced. The staging and imaging findings of the PHPT were determined by using MRI and spiral CT scan and comparied with pathological of the bone disease. At the same time, the bone mineral density (BMD) were performed by quantitative computer tomograph ( QCT) , which is simplicity and equivalent accuracy in diagnosing osteoporosis. We performed the BMD in osteopathy by QCT and determined the apoptosis and proliferation of parathyroid cell in PHPT, and tried to draw a conclusion that the BMD of bone disease correlated with apoptosis and proliferation in parathyroid cell in PHPT.Materials and methods1. Establishment of animal model with primary hyperparathyroidism (PHPT). 120 Chinese rabbits were divided into 12 groups of 10 each, and fed a control diet(Ca;P, 1:0.7) in control groups(60 rabbits) or a high-phosphate diet(Ca:P, 1:7 ) in experimental groups(60 rabbits) for 1-, 2-,3-,4-,5- , or 6-month intervals, both ad libitum. Both groups also received deionized water to drink ad libitum. Control and experimental groups were then sacrificed at l-,2-, 3-,4-,5-,6-month intervals and terminal blood samples were obtained. The thy-roparathyroid complex was removed en bloc, fixed in neutral formalin, and prepared for histologic examination. The kidneys were also removed and similarly prepared for histologic examination. The skeleton of imaging study and pathological examination were performed in these rabbits regularly.BiochemistryCalcium. Total serum calcium determinations were determined with reagents from Shanghai Kehua Chemical Co. according to the colorimetric procedure of Gitelman.Phosphorus. Serum inorganic phosphorus levels were determined with reagents from Shanghai Kehua Chemical Co. by a modified method of Daly and Ert-inghausen.PTH. Rabbits serum PTH was determined by a radioimmunoassay developed in our laboratory. This method uses human PTH as a standard. This assay has been validated several times during the past 20 years.Hist...
