Part I : Clinical observation of treatment with manuever therapy relaxing thesternocleidomastoid muscles on Cervical spondylopathyObjective:To observe the effectiveness of treatment with manuever therapy relaxing the sternocleidomastoid muscles on Cervical spondylopathy, and discuss the relationship between the sternocleidomastoid muscles relaxing and Cervical spondylopathy curative effect. Methods:90 cases of Cervical spondylopathy were selected in outpatient and in-hospital patients of WangJing Hospital and Orthopedics and Truamatology Department form December 2003 to December 2005. These cases were divided into three groups randomly: manuever therapy relaxing the sternocleidomastoid muscles group, composite manuever therapy group and contrast group. All the cases are analysised statitiscally with no significant discrepancy ( P >0.05 ) in sex, age, and disease history which has senses to compare.There are three main clinical targets : manuever therapy curative effectiveness, cervical radiographs and serum enzymology. Results:1 The whole effectiveness of the treatment groups with manuever therapy relaxing the sternocleidomastoid muscles and composite manuever therapy is obviously superior to the contrast group (P<0.05) .which shows that the treatment with manuever therapy relaxing the sternocleidomastoid muscles is also an effective method to Cervical spondylopathy, though the later is more effective.2 Cervical Radiographs analysis:The improvement of cervical radian, function range and Atlas-axis arthrosis of the treatment groups with manuever therapy relaxing the sternocleidomastoid muscles and composite manuever therapy is obviously better than the contrast group. (P<0.05, P<0.01).3 Serum enzymology analysis:The level of CK amount of the two tre_ atment groups is higher than the contrast after one course of treatment, but has no significant discrepancy (P>0. 05 ) .and the level of LDH amount of that is obviously lower the contrast group. It shows that manuever therapy can regulate changes of CK and LDH, which may be a target of cervical spondylopathy during diagnosis and treatment. Conclusions:The treatment with manuever therapy relaxing the sternocleidomastoid muscles is an effective method to Cervical spondylopathy, if being combined to manuever therapy of sun surname is more effective. The sternocleidomastoid muscles is one important muscles to Cervical spondylopathy wchich should be emphasis point of manuever therapy.Part II :The related experimental study of rabbit' sternocleidomastoid muscles shortened leading to Dynamic dysequilibrium Objective:To estabish a new animal model of rabbit' sternocleidomastoid muscles shortened leading to Dynamic dysequilibrium and discuss the relationship between the sternocleidomastoid muscles shortened spasming and Cervical disc degeneration.1 To estabish an new animal model of rabbit' sternocleidomastoid muscles shortened leading to Dynamic dysequilibriumMethods:All research forty rabbits were selected weigheted form 2. 5Kg to 3. OKg and divided radomly into four groups, A, B, C and D group. The four group are all shortened sternocleidomastoid muscles to to estabish the new animal model, in which A and B groups are fixed the senor for special mechanics measurement purpose. Results:The muscles strength of the rear cervical muscles of A group which are examined at allthe time points in a week is obviouly higher than that of B group ( P< 0.01).and at the same time the sternocleidomastoid muscles' has significant changes. The Cervical Radiographs and Serum enzymology(CK and LDH) of C and D groups are also has significant changes after two months ( P< 0.05). It' s successful to establish the model of rabbit' sternocleidomastoid muscles shortened leading to Dynamic dysequilibrium.2 Study of pathology changes of the Cervical muscles and discs of the model of rabbit' sternocleidomastoid muscles shortened leading to Dynamic dysequilibrium.The method of establishing animal model is same to the experiment 1. Results:The cervical muscles and discs of model groups has a serise of obviously pathology changes , and the amounts and areas of muscles fibre is much lower than the compared group ( P<0. 05), which shows that the sternocleidomastoid muscles if being shortened for a long time can lead to Dynamic dysequilibrium, even to Cervical muscles and disc degeneration.3 Experiment study on the a -skeleton muscles actin changes in the Cervical muscles of the model of rabbit' sternocleidomastoid muscles shortened leading to Dynamic dysequilibrium.Form the angle of immunochemistry of tissue, to observe the a -skeleton muscles actin changes in the cervical muscles.Results:The a -skeleton muscles actin content of the model group in the cervical former and rear muscles is obviously lower than the compared group ( P<0. 05). It can conclude that cervical muscles degenerate in it' s early stage after sternocleidomastoid muscles shortened for a long period.4. Study of the Apoptosis in the Cervical muscles and discs on the model of rabbit' sternocleidomastoid muscles shortened leading to Dynamic dysequilibrium.Form the cell level, to study the Apoptosis changes in the cervical muscles and discs.Results:The amount and index of Apoptosis cells in the muscles and Cervical discs of the model is significant lower than the compared group, and also found that the Apoptosis comes out in fibrous ring more than other area which shows cervical discs degeneration may occur in it' s early stage, the sternocleidomastoid muscles if being shortened for a long time can accelerate its happening.Conclusions:The new animal model is successfully estabished thought rabbit' sternocleidomastoid muscles shortened leading to Dynamic dysequilibrium. The muscles and cervical discs of model groups has a series of obviously changes on pathology, the content of a-skeleton muscles and the Apoptosis. All the results shows that cervical muscle has closely relationship with Cervical composition degeneration which should have the doctor' attention highly.
