Objectives: To investigate antimicrobial resistance in Acinetobacter species from 10 teaching hospitals in China; To investigate molecular epidemiology of carbapenem-resistant Acinetobacter species; To study the molecular mechanism of resistance to carbapenems; To determine the role of β-lactamases, integrons, outer membrane proteins (OMP) and efflux pumps in mediating the resistance to multiple drugs or pan-drugs among Acinetobacter species.Methods: Non-repetitive 187 carbapenem-resistant isolates of Acinetobacter baumannii were collected from 10 teaching hospitals from 2003 to 2004. Agar dilution method was used to determine the minimal inhibitory concentration (MIC) of meropenem, imipenem and others antibiotics. 128 carbapenem-resistant Acinetobacter baumannii (CRAB) clinical isolates were collected from 7 big hospitals from 1999 to 2004. The homology of the isolates was determined by both pulsed field gel electrophoresis (PFGE) and randomly amplified polymorphism DNA (RAPD). Retrospective analysis were conducted for the outbreak due to carbapenem-resistant Acinetobacter baumannii at Peking Union Medical College Hospital. Ten diffferent clone strains were selected for further study. Analytical isoelectric focusing was used to measure the pI of the β-lactamase and inhibitory testing to determine the type of enzymes. Plasmid DNA was extracted and purified Qiagen Plasmid Mini Kit. blaTEM-- blaSHV, blaPER, blaCTX-M blaVEB, blaGES blaIMP, blaVIM, blaOXA23 suhgroup blaOXA.24 subgroup genes for these clone strains were amplified and sequenced.
