Part One Clinical study on the Modified Classification of Type A Aortic Dissection and the StrategyObjective: We retrospectively analyzed the early operative outcome of Stanford type A aortic dissection between January 1994 and December 2004, summarized the optimal operative management of type A aortic dissection and relational problems. The determination of optimal operative procedures, indication, plan and the estimation of the prognosis depended on the pathological classification of the proximal aortic dissection, the transverse arch and the distal arch. Methods: 477 patients who suffered Stanford type A aortic dissection were treated. Type A dissection can be classified into 3 subtype based on the pathological change of the aortic root. Type A1 (No pathological change type): 212 patients underwent ascending aorta replacements (type S) or plus arch replacement with elephant trunk procedure (type C). Type A2 (Mild pathological change type): Sixty three patients underwent ascending aortic replacement with concomitant aortic valve and Valsalva sinus plasty.
