The breast conservative surgery for breast cancer has been popular gradually in many European and American countries,but in China the therapy still remain the beginning stage. Breast conservation therapy (BCT) is a very compound problem in theory and practice . It relates to the development of the theory of breast cancer local treatments , the progress of the combined therapy . BCT could give equal survival rate with mastectomy and better local control results, at the same time it reserves the shape of the breast, decreases the psycholgical problems, improves the quality of life. In order to get the better cosmetic results,we retrospectively analysed the date on 67 patients with 67 breast cancer,who had been admitted to Peking Union Medical Hospital Breast Center between January 1987 and January 2004 . Twenty-eight kinds of possible factors affecting cosmesis were grouped as patient-related, tumor-related and treatment-related. Mann-Whitney U test was used in univariate analyses whereas logistic regression was used in multivariate analyses. The follow-up time ranged between 6 months and 15 years after all of the treatments had been finished, the patient's age ranged between 22 and 78 years old. Treated breast were scored by the plastic surgeon and the patient independently. Cosmetic results were grouped in four categories: excellent, good, fair, poor by the surgeon, using criteria ,such as fibrosis, telangiectasia, wound scan pigmentation, the deformity of the axilla, the defect of the skin and the gland, the displacement of the nipple-areola, symmetry, requiring additional operation.
