BADKGROUND:The outcome of liver transplantation may mostly depends on quality of liver, extent of ischemia-reperfusion injury(I/R)and rejective reaction. Apoptosis is a central mechanism of reperfusion injury in grafted liver. The secondary damage to the grafts may always go with apoptosis in liver transplantation while the apoptosis index correlates with the degree of liver insult. That outer antiapoptotic genes introduced into liver express functional protein in transplantation may have play an important role in alleviating posttransplant immune reactions and graft ischemia-reperfusion injury .Effective gene therapy requires a reliable method of gene transfer that efficiently inserts target genes to the parenchymal and nonparenchymal cells in grafts to produce required levels of proteins. Adenovirus have an ability to readily infect nonproliferating cells in liver. Although adenoviral-mediated recombinant gene expression has been limited to a short period after gene transfer, this approach may have the advantages in attenuating the critical early posttransplant damage (intensive ischemic reperfusion injury ) and be an attractive avenue to improve outcome of the transplantation.PART I:Objective:To establish an animal model of rat orthotopic liver transplantation with modified cuff technique.Methods:150 cases of rat orthotopic liver transplantation were performed by using the two-cuffed technique with some modification in graft harvesting, recipients anesthesia, the anastomoses of suprahepatic vena cava and the treatment.Results:Of the last 30 cases , the mean time of an hepatic period was 13 min, and the mean operation time was 45min.
