Objective: ①To study intramedullary pressure(IMP) change during open intramedully nailing(OIMN) of fresh femoral fractures and the factors leading to the change. ②To study fat intravasation and its relation with IMP during OIMN of fresh femoral fractures. ③To evaluate the effect of medullary cavity clearing and vacuum sucking on fat intravasation during OIMN of fresh femoral fractures. ④To estimate the value of transcranial Doppler ultrosonography(TCD) in monitoring fat intravasation during OIMN of fresh femoral fractures. ⑤To observe the dynamic change of pathological detection for free fat in blood ,TCD microembolus monitoring and cranial MRI in early stage of fat embolism syndrome(FES). ⑥To estimate the value of TCD in the prediction and early diagnosis of FES.Methods: The first part: ①50 patients with femoral fractures were submitted to OIMN in early stage after injury. ②Before operation, a hollow probe was screwed into the medullary cavity and linked with a pressure monitor to record the pressure change during the OIMN process. ③5ml blood was taken from femoral vein of the affected limb individually before operation and just after fixation of intramedullary nail.
