Patients who can not weaning cardiopulmonary bypass (CPB), can support with left ventricular assist.It is easy and familiar to us to use BioMedicus centrifugal pump.Medical cost is lower with centrifugal pump than with pulsatile pump.It can spread nationalwide quickly.There are 2 kinds of method to establish left heart assist (LHA). One method to establish LHA needs thoracoty, which may, rethoracotomy to wean LHA, cause bleeding and infection. The other method is through percutaneous puncture into peripheral artery or vein to perform LHA, avoiding rethoracotomy when weaning LHA. But,it need x-ray machine or transesophageal echography.We combine the 2 methods, establishing LHA under CPB, because patients who need can not weaning CPB.Aiming at avoiding rethoracoty, we insert inflow cannula from femoral vein to right atrium,then cut right atrium and interatrial septum 4 x 4mm, send inflow cannula through the pore into left atrium. And outflow cannula is insert into graft anastomosis with femoral artery. The method is safe, easy and quickly to perform LHA, suit for patients who can not wean CPB.With centrifugal pump, 4 patients performed LHA. 2 of them support with the method and the other 2 patients with left atrial draining tube and aorta intubate. 3 patients who performed coronary artery bypass graft survive and 1 patient after remitral...
