ObjectivesThere are numerous posterior fixaion technique for thoracic spine, posterior transpedicular screws fixation produced a three-point fixation and offered a best biomechanical stability. However,the transpedicular screw technique is technically demanding and requires considerable experience. Because the thoracic pedicle is thinner than lumbar pedicle anatomically,and the spinal cord is in the thoracic spinal canal,transpedicular screw fixation in thoracic spine is more dangerous and difficult than lumbar spine,complications as neural-vascular injury is encountered sometimes. Particularly in some cases of scoliosis or other spine deformity,the morphology of pedicle deformed severiosly and the anatomic relationship between pedicle and around structures also change,it is much more dangerous and difficult if placing an transpedicular screw in this situation. The extrapeicular method of costre-transverse approach offered a good fixation method for thoracic posterior fixation,it is safer than tranpedicular method and possess much advantages.but the costre-transverse method is with overelaborated penetration angle...
