| This research consists of 5 parts that include: relatively systematic sorting out and analyzing ancient and modern documents about asthma, forming the prescription of Yichuan capsules, partly experimental study of Yichuan capsules, small sample clinical treatment on Heat Asthma and conclusion. All of prescription, experimental research or clinical study is based on the Qingfei, Huatan, Pingchuan rule of TCM (Traditional Chinese Medicine).1 Through relatively systematic sorting out the ancient and modern documents about asthma, radically analysis of its origination, development, cure and existing situation of treatment by any TCM, western medicine, or combination of TCM and western medicine is performed.Ancient documents on Heat Asthma (asthma caused from accumulated heat in lungs) demonstrate that is a result of many factors with syndromes of counterflow rise of lung qi. Lung qi failing in purification and descent brings excessive phlegm to obstruct respiratory tract. Successively the phlegm brings about gasp for breath or even difficulty in breath. Among Heat Asthma, there are 3 main types that called syndrome of wind-heat invading lung, syndrome of phlegm-heat obstructing lung, fire-toxicity syndrome.Under TCM theory, the basic rule for Heat Asthma cure is established to Qingfei (clearing heat in lung), Huatan (dissipating phlegm), and Pingchuan (relieving gasp for breath).In modern research on Heat Asthma, a wide-range happened but high dangerous disease called acute stage of chronic asthmatic bronchitis is considered approximately the same with syndrome of phlegm-heat obstructing lung in TCM. Whereas, treating method for acute stage of chronic asthmatic bronchitis emphasized to reduce trachea inflammation. This happened to coincide with a basic treatment rule of TCM which says"to alleviate the symptoms of an illness during acute situation".Joint prescriptions consist of TCM and western medicine demonstrated comparatively good therapeutic effect than routine treatment of western medicine or either single TCM in asthma cure. Combination of TCM and western medicine can also take advantage of a coordinate action of TCM that possibly attend to patient's intrinsic illnesses such as hypertension, diabetes, etc.. Otherwise, TCM has comparatively little side effect however has reliable curability.So it is significant to conduct objective research on Heat Asthma treatment with TCM. 2 Forming of the prescription of Yichuan capsules is a result of integrating TCM ancient theory heritage with personal clinical experiences.Yichuan capsules was prescribed considering the pathogenic factors and pathogenesis of heat asthma and following treatment rule under TCM theory. Clinical medicine choice experience of predecessors and myself personal asthma treating knowledge so far were also organically combined in this prescription. This prescription was also assisted by positive results of pharmacological research of TCM.Aimed at heat-phlegm obstructed in respiratory tract, this prescription uses Zhudanzhigao, etc. to clear internal heat in lungs, to eliminate the block of phlegm in respiratory tracts, to relieve inflammation as an antipyretic and to resolve gasp for breath. It was a general diagnosis and treatment based on an overall analysis of the illness and the patient's condition.Yichuan capsules was an exclusive and curative prescription for Heat Asthma.3 Through experiment research to test the pharmacological action and safety of Yichaun capsules.Because Yichuan capsule is directly against syndrome of phlegm-heat obstructing lung and has efficacy to relieve cough, excessive phlegm and gasp for breath, so that experiment of relieving cough of model mouse and guinea pig, anti-inflammation of model mouse and rat, making expectoration easy on model mouse, relieving gasp for breath of guinea gig, in vitro bacteriostasis were designed.The doses applied to mouse were 0.5g/kg, 1.0g/kg, and 1.5g/kg and to guinea pig were 0.35g/kg, 0.7g/kg, and 1.4g/kg.All the conducted experiments had given positive effects. These results indicated that Yichuan capsules was reliably effective in cough, expectoration and gasp for breath.Yichuan capsules was given to rat with doses of 6g/kg, 1.4g/kg, and 0.7g/kg for 90 days. The highest dose was 100 times compared to people and the treatment period was 13 times to clinical medication.Comparison between control group (applied water instead of Yichuan capsules) and medication group (applied Yichuan capsules) showed that there was no distinct deference in blood, blood chemical indicators and pathological histology observed. These results showed that Yichuan capsules was nontoxic and safety.4 Small sample clinical study of Yichuan capsules.Clinically compare Yichuan capsules with Jinbei Tankeqing granules on symptoms (cough, expectoration, gasp for breath, wheezing) improvement gave signs that Yichuan capsule had better efficacy than Jinbei Tankeqing granules with effective rate 93.34% and 80% separately. The two prescriptions have almost the same effectiveness in cough improvement however Yichuan capsule showed higher effective rate in expectoration, gasp for breath and wheezing. There was no any side effect observed.Clinical experiment comparison between Yichuan capsules and Jinbei Tankeqing granules regarding peripheral WBC (white blood cell), complement C3, PGE2, TXB2, IgG, IgA and IgM gave signs that the two prescriptions have similar anti-inflammation effect and immunity enhancement ability. Both can adjust microcirculation of blood in lungs so as to improve lungs function. But in relieving bronchial spasm and respiratory tract obstruction Yichuan capsules showed more obvious cure effect.5 Conclusions1) By analyzing ancient and modern documents about asthma, it became clear that Heat Asthma is often observed to be a syndrome of phlegm-heat obstructing lung in patients which is considered approximately the same with modern acute stage of chronic asthmatic bronchitis. For that, under TCM theory, the basic rule for Heat Asthma cure is established to Qingfei (clearing heat in lung), Huatan (dissipating phlegm), and Pingchuan (relieving gasp for breath).2) Reflecting an organic combination of ancient theory heritage and clinical practice experiences of prescription, Yichaun capsules offered an exclusive and therapeutic effect prescription for Heat Asthma. 3) Pharmacological experiments gave reliable evidence of Yichuan capsules on improving cough, expectoration, relieving gasp for breath and anti-inflammation. Also gave an effect proof on bacteriostasis. These results indicated that it is applicable to clear heat in lungs of patients suffering Heat Asthma. Furthermore, this prescription is rather safe regarding no matter its medication dose or treatment period.4) Small sample clinical study showed that Yichuan capsules had definite therapeutic effect for acute stage of chronic asthmatic bronchitis, which also can be called Heat Asthma. Its cure principle keeps closely to the point of pathogenesis of Heat Asthma and applicable to almost all cases of Heat Asthma. Besides, its immunity enhancement effect will bring good affect to the stable condition of patients later after their acute stage being relieved.
