Shengmai Capsule For The Mechanism Of Necrosis Of The Femoral Head Within The Blood Vessel Regeneration

[Aim] To investigate the mechnism of the lesion, regeneration and recovery of blood vessel in the procedure of non-traumatic ischemic necrosis of the femoral head, to explore the effective way of shengmaichenggu capsule promoting blood vessel regeneration in the necrotic femoral head and to demonstrate the effect of shengmaichenggu capsule promoting the plerosis of ANFH by ways of angiogenesis.[Content] Firstly, we picked up some discriptions in the traditional Chinese medicine theories about "Qi and Xue"and "Blood stasis" and explained that "Qi and Xue" were the basis of the normal function of "Five Zang "and "Six Fu". Additionally, we reviewed the anatomy of blood supply system in the skeleton system and the structure of microcirculation in the femoral head and described the research development of the mechanism of non-traumatic INFH on the relationship between blood supply deficience and INFH, which can explain that INFH results from many diffirent depended factors. Then, we summarized the research development of angiogenesis and the past data of shengmaichenggu capsule promoting angiogenesis. In the animal experiments, We researched the blood vessel pathology in necrotic femoral head by such two models as overdose Corticosteroid and bacteria endotoxin plus Corticosteroid ; The confirmed non-traumatic INFH models were treated by shengmaichenggu capsule and the therapeutic effects wereevaluated majorly from blood supply system. The indexs include pathology, biologicalchemistry, serum examinatin, iNOS immunohistochemistry, VEGFmRNA hybridization in situ and so on. In the end, data were disposed by statistics. .[Result] The results of literature investigation show that the "Qi and xue" are the basis of normal function of body and "Blood stasis"are the reason and result of the abnormal functions in traditional Chinese medicine. Intravessel coagulation (1C) is an important pathological mechanism for non-traumatic INFH. The synthetic factors including 1C, the special anatomy of blood supply system and the structure of microcirculation in the femoral head and so on result in the ischemia and necrosis of the femoral head. Angiogenesis give a promise to the recovery of blood supply of the necrotic femoral head and the past researches proved shengmaichenggu capsule had a good effect to vessel regeneration and recovery. The serum examination results show that 6-keto-PGFl a are getting higher with time shifts, but TXB2 and T/G lower in those animals performed shengmaichenggu capsule, which accompanied with the changes in the pathological obeservation allude the herbs can correct blood high coagulation and low thrombo-dissolving situation of non-traumatic ANFH and prevent the generation of 1C and thrombi in the blood vessel. The iNOS immunohistochemistry stain and hybridization in situ expression of VEGF mRNA in the angio-endothelial cells, osteoblasts and chondrocytes in the femoral head imply the active angiogeneration and bone plerosis, which can certify shengmaichenggu capsule can promote angiogenesis by modulating the expression of such cytokinins as iNOS and VEGFmRNA.[Conclusion] shengmaichenggu capsule is used to promote the regeneration and plerosis of blood vessels in the necrotic femoral head, which not only are supported by TCM and modern medical theories but also have a good research base. In the plerosis of non-traumatic INFH, shengmaichenggu capsule can promote angiogenesis by ways of elimilating the 1C situation, modulating the expression of such cytokinins as iNOS and VEGFmRNA. The adequate blood supply can promote the absorption of necrotic bone and new bone formation so as to prevent and treat non-traumatic ANFH.
