Diabetic nephropathy (DN) is one of the main chronic complicants of diabetic mellitus (DM) as well as the common reason resulting in chronic renal failure, one third of which is caused by DN. Systematic studies on the prevention and treatment for DN by JieDuTongLuoBaoShen capsule (JDTLBS capsule) had been performed from the aspects of reference reviews and experimental study in this essay1. Reference review1.1 the skeleton research of traditional Chinese medicine (TCM) on DN This essay reviewed from the advances of such fields as followed: the source and name of TCM for DN, etiology and pathgenesis, selection of treatment or by different stage based on the differential syndrome diagnosis, the treatment of the single herb or its abstract, moreover, included the evaluations and prospects on the criterions of diagnosis and curative effect as well as the problems lying in the selection of treatment based on the differential syndrome diagnosis or scientific study.1.2 the latest experimental study of TCM on DNthis essay summarized the experimental study results and advances on the principle of preventing and treating DN, according to the pathologenesis of DN, such as glucoregulation tubulence, fat matabolism tubulence, vascular active substance, growth factor, chemotactic factor, reactive oxygen stecies, experimental animal model and so on. It also included evaluations and prospects on these above.1.3 renin angitensin system (RAS), transforming growth factor-β (TGF-β), connective tissue growth factor (CTGF) and the pathogenesis of DNThe results and progresses of study on the RAS, TGF-β1, CTGF were summarized involving the pathogenesis of DN recant years.2 Experimental study2.1 the effect of JDTLBS capsule on renal functions of experimental diabetic rats STZ-induced diabetic rats were divided randomly into normal control group, diabetic control group, postive control group (DM+Benazepril), experimental group (DM+JDTLBS capsule). The various markets were observed at the end of 12 weeks. The results showed JDTLBS capsule was better for the rats' general condition and had a certain effect on reducing blood sugar and serum cholesteroal, which had a significant statistic sense in comparing with diabetic control group (P<0.01, P<0.05). Besides it could inhibit renal hypertrophy in early stage and high filtration of renal glomerulus as well as decrease the experimental rats' higher secrum BUN, Cre, Ccr, which had a significant difference against diabetic control group (P<0.01, P<0.05). It also reduced the experimental rats' urinary al bumin excretion rate (UAER) and urinary protein amounts in 24h, having a markedlydifference against diabetic control group (P<0.05, P<0.01). The results indicated that JDTLBS capsule was benefit for regulating glucose and lipid metabolism, inhibiting renal hypertrophy and high filtration of renal glomerulus, reducing the urinary protein excretion, improving renal funition.2.2 the effect of JDTLBS capsule on the changes of renal tissue morphology in experimental diabetic ratsThe changes of renal tissue morphology in experimental diabetic rats were observed by optical microscope and electric microscope. What were seen by optical microscope that mesangial area of renal glomerulus broadening, even protein substances with PAS postive markebly increasing, extracellular matrix of glomerulus enlarg ing in number, glomerular basement membrance (GBM) thickening and so on.The results observed by transmission electric microscope showed that GBM had been thick, some of which were obscure, and epithelium hernia mixed in full or a part, besides, the hiatus of filtering membrane broadened, so did mesargial area, and the number of mesangial cell increased.The pathlogical changes of experimertal group treated by JDTLBS capsule for 12 weeks allieved more obviously than diabetic control group, The JDTLBS capsule effect on the deposit of collagenⅣ (colⅣ) and fibronectin (FN) in diabetic rats was observed by immunohistochemisty method.
