OBJECTIVE: To find out the etipathogenesis of trigeminal neuralgia,and research the principles and methods of surgical treatment, we selected clinical data of 110 trigeminal neuralgia patients, the manner of operation, finding of operation, effectiveness and complications of treatment were discussed.METHOD: Between 1990 and 2005, the data of 110 patients with trigeminal neuralgia were selected . The ages of this group are from 19 to 72, 49.4 in average;and they include 45 male and 65 female. The course of disease are from 5 months to 20 years ,7.9 years in average. All patients were treated surgically,.Of them, 65 were operated through postsigmoid sinus approach, 45 were operated through postsigmoid sinus and Small Bone Window approach.21 were treated with partial sensory rhizotomy,89 were treated with microvascular decompression.RESULTS:In operation, 79 trigeminal nerves were observed to have been in touch withvascular, accounting for 71.82%. There were 51 SCA, 13 AICA, 1 AL. In 5 patients, SCA and AICA were found together. 9 trigeminal nerves were found to be contacted with the vein. In 12 patients we found arachnoid conglutinated with trigeminal nerve. Abnormity was not found in 9 patients.In 21 PSR, pain has been eliminated in 16cases, alleviated in 3 cases. In 89 microvascular decompression, pain has been eliminated in 79cases, alleviated in 7 cases.Conclusino:(1) Multi-facts influenced sence root of trigeminal nerve in PC A is a primary cause of trigeminal neuralgia.(2) The operation through postsigmoid sinus and Small Bone Window approach and microsurgical operation can reduce the postoperative complications of operation.(3) PSR were performed when trigeminal nerves were found to be contacted with the vein , arachnoid was conglutinated with nerve, and abnormity was not found around trigeminal nerves.(4) Microvascular decompression is safe and effective treatment for trigeminal neuralgia.
