Hydrochloric Guanfu base A (GFA) is a new anti-arrhythmic alkaloid with a structure of C20-diterpenoid isolated from the tuber of Aconitum coreanum (Levl.) Raipaics. In pre-clinical studies it was shown to slow conduction velocity in nearly all compartments of the heart in vivo and in vitro and exhibited anti-arrhythmic effects in various experimental arrhythmia animal models. After finished phase I to III clinical trails, GFA has been approved by SFDA to treat some arrhythmias. Most of the participants in the clinical trials, however, were those with mild structure heart disease. This is not the case in emergency arrhythmias in which myocardial ischemia or cardiac insufficiency is complicated. It is necessary to evaluate the effect and safety of GFA in this setting.
